Oral ulcers caused by hydroxyurea

Abstract: We present a case of oral ulceration in a patient with essential thrombocythaemia, that proved to be secondary to treatment with hydroxyurea. This complication has not yet been reported in the ENT literature. Hydroxyurea should be considered as a possible cause of persistent oral ulceration in patients with haematological conditions. It should be noted that the onset of this complication is not related to the dose and duration of therapy.

“…Other common side effects are gastrointestinal (GI), such as nausea, vomiting, diarrhea, anorexia and stomatitis, as well as cutaneous, such as skin hyperpigmentation, nail changes and alopecia [3,4]. Unusual mucocutaneous side effects are leg and rarely oral ulcers, which may occur after a prolonged use, and are reversible after drug cessation [5-8]. Leukocytoclastic vasculitis was shown in biopsies of leg ulcers from patients with chronic myeloproliferative neoplasm who had been on this drug [9-11].…”

“…3 The rate of intolerance to hydroxyurea due to side effects has been reported to be between 5% and 16%. [4][5][6][7][8][9] The oral cavity is by far the commonest site for ulcer formation in the gastrointestinal tract associated with hydroxyurea, [10][11][12][13] and oral ulcers were reported to occur in 0.8% of patients with myeloproliferative neoplasm on hydroxyurea therapy. 8 Ulcers in the other parts of the gastrointestinal tract are exceedingly rare and to our knowledge, there have been only 3 such reported cases as summarized in Table 1.…”

“…Sie sind meist schmerzhaft, fibrinbelegt, haben einen atrophischen Rand und sind durch eine ausgeprägte Therapieresistenz gekennzeichnet. Orale Ulzera sind eher selten [20]. In Einzelfällen wurden Ulzerationen auch genital [16], postoperativ [30] und an den Handrücken [12] beschrieben.…”
