Os acromiale: frequency and a review of 726 shoulder MRI

Abstract: Os acromiale consist in a lack of fusion between the different ossification spots of the acromial side of scapula from the age of 23-25 years. A relation between os acromiale and some shoulder pathology like impingement syndrome, cuff tear and subacromial bursitis has been described. The etiology is not already known. The aim of this study was to evaluate the frequency of os acromiale in our population, the link between os acromiale and sex, side and shoulder pathology. 1042 shoulder MRI were evaluated to find… Show more

“…12 The most recent evidence using MRI imaging in a cohort of 1042 patients suggests a lower frequency of 3.4%. 13 Examination of Table 1 suggests a spread of frequency from 2.7–15.0% with generally higher frequencies seen in anatomical studies. Of note, Sammarco reports a significant demographic variance.…”

“…Both anatomical and radiological studies examining the frequency of os acromiale exist in the literature (see Table 1). 5–13 Early anatomical studies from Gruber, and Macalister suggest an incidence ranging from 3% to 15%. 5,6 In the modern literature, Sammarco suggests an incidence of 8% from the examination of 1198 scapulas from the Hamann–Todd osteological collection in Cleveland, USA.…”

“…In an adolescent patient presenting with a painful shoulder, it may be difficult to distinguish a normally developing acromion with a secondary ossification center from the early formation of an os acromiale on the basis of age alone. Therefore, MRI and SPECT–CT are receiving more and more attention.
