Abstract: Intraocular pressure was well controlled by trabeculectomy; however, a steady decline in intraocular pressure control, visual acuity and visual field occurred during follow up.

“…An accepted method of maintaining aqueous drainage has been conventional trabeculectomy where, after raising a conjunctival flap, an artificial channel is made between the anterior chamber and the subconjunctival space by means of a scleral flap [1]. Reduction of intraocular pressure (IOP) has been known to be sustained by this channel over a period [69]. In this paper, we have presented a modified technique of trabeculectomy for primary open angle glaucoma (POAG) where a scleral tunnel has been used with decreased conjunctival dissection.…”

“…39 The reported reoperation rate following initial trabeculectomy was 4.8% at 6.7 years and 8% at 7.5 years. 40,41 In China, the success rate of primary trabeculectomy in PACG has been reported as 90% to 95% over a period of 2 years. 42,43 A large retrospective study in Korea examined 900 patients (870 POAG eyes and 132 PACG eyes) to determine the 15-year success rate.…”

“…Hyphema (27%), 40,59 hypotony (24.3%), 59 shallow anterior chamber (23.9% 59 to 40% 46 ), and choroidal detachment (12% 40 to 14% 59 ) are some of the early complications. Long-term complications include cataract (20% 54 to 50% 55,56 ), scarring of filtering bleb (3.4% to 16% 40,41,59 ), and bleb-related infection (1.3% to 2.0%). 60,61 In addition, the surgery impairs visual acuity during the early phase in about 20% of the patients.…”

“…Orbit content has importance for IOP value: in Graves disease muscle thickness can increase to compress the bulb, giving rise to a constant increase of pressure. 20 In NTG patients, the favorable effect of IOP reduction on visual change progression was only found when the impact of cataracts on visual field progression was removed. However, results are sometimes controversial.…”
