Introducción: Las infecciones respiratorias virales constituyen una problemática nacional y las unidades de cuidados intensivos neonatales, no están exentas de estas. Objetivo: Describir la frecuencia y características clínicas de las infecciones respiratorias virales en neonatos internados en un servicio de Neonatología durante el periodo de enero 2017 a octubre 2018. Materiales y Métodos: Estudio observacional, descriptivo, retrospectivo. Fueron analizadas historias clínicas de pacientes hospitalizados en la unidad de cuidados intensivos neonatales, que tuvieron diagnóstico de infecciones respiratorias de origen viral, el método de diagnóstico fue inmunofluorescencia directa, inmunocromatografía y PCR en tiempo real. Se excluyeron pacientes que procedan de otro servicio y hayan iniciado los síntomas antes del ingreso a nuestro centro. Variables: edad gestacional, edad posnatal, peso de nacimiento, síntomas, tipo de virus, prescripción de antibióticos. Los datos fueron analizados con Excel, utilizando estadísticas descriptivas. Resultados: De los 761 pacientes, 65 pacientes presentaron diagnóstico de infección respiratoria viral: frecuencia del 8,5%. EL promedio de edad fue de 17±10 días El método de diagnóstico más frecuente fue inmunofluorescencia 92%, Los síntomas más comunes fueron las secreciones respiratorias (74%) y desaturaciones (64%); Los virus más frecuentes fueron: Sincitial Respiratorio 26%, Influenza B 25%, Influenza A en un 18%. El 66% requirió tratamiento de soporte respiratorio. Un paciente falleció por causas no relacionadas a la infección respiratoria viral. Conclusiones: La frecuencia de infecciones virales fue del 8,5%. Los virus más frecuentes fueron VSR e influenza B. Más de la mitad necesitaron soporte respiratorio. Ningún paciente falleció.

Comparative Study for Probiotics and Symbiotics Versus Placebo in Pediatrics Acute Diarrhea: Randomized Controlled Trial

Background: Acute diarrhea is a major cause of morbidity and mortality in children, particularly in developing countries (the second cause of death). Probiotics and symbiotics are recent treatments for this disease, especially in the acute phase. Our objective is to compare probiotic or symbiotic treatment against placebo in acute diarrhea by following the evolution of diarrhea in terms of hours, number of bowel movements, volume of stools and their consistency. On a larger scale, we want to find a cost-effective intervention that reduces the morbidity and mortality of diarrhea. Methods: Eighty-three children aged 6 months to 5 years, from three different regions of Mount Lebanon, were randomized to receive a probiotic, a symbiotic or a placebo once daily for 5 days. Patients were excluded from the study if they had any history of a chronic disease. The statistical analysis was carried out on SPSS v22.00 Results: Out of 120 surveys distributed to parents, 84 were completed: 43 patients received probiotics (nine received Lactobacillus, 21 received spores, and 13 patients received yeast), 24 received symbiotics and 17 were controls. Stool consistency normalized on day 4 in the probiotics and symbiotics groups (P = 0.009). Less number of days with fever (P = 0.018) were observed in the probiotic and symbiotic groups (1 day) compared to placebo (4 days). No difference in the symptoms associated with diarrhea was observed in the different groups. Conclusions: Probiotics and symbiotics normalized stool consistency in pediatric diarrhea by day 4 and decreased the number of days with fever compared to control. Our study did not show a statistically significant difference between the different probiotics and symbiotics for the treatment of diarrhea.

Professor Tina Dalianis, Professor of Tumour Virology at Karolinska Institutet in Stockholm, Sweden, is a well-known virologist with a strong international reputation and seminal contributions in tumour virology in the fields of human papillomaviruses (HPVs) and polyomaviruses. According to Professor Dalianis, over the past decades virology has significantly contributed to the understanding of many infectious diseases in medicine. In addition, quite a few antiviral therapies have been developed in recent years that have been very helpful, while the study of viruses has also provided valuable information about their host cells and basic cell biology. She highlights the association of HPVs with tonsillar and base of tongue cancers, as well as their epidemic rise. She also refers to additional biomarkers, which are under investigation, in order to better individualise patient care, by for example, de-escalating or targeted therapy, whenever this is possible. She clarifies that the current prophylactic vaccines against HPVs are very efficient for the types they are directed against and she hopes that boys will also soon be vaccinated against HPVs, particularly in countries where there is a low percentage of girls being vaccinated. According to Professor Dalianis, the knowledge of virology is very useful in general paediatric practice; the proposal of paediatric virology as a medical subspecialty is absolutely correct and it would be very useful, as there is much to be done. In the context of the ‘4th Workshop on Paediatric Virology’, Professor Tina Dalianis will give the plenary lecture of this scientific event and will focus on changes in the prevalence of different HPVs in genital and oral samples in some groups of youth in Sweden during the introduction of the vaccines against HPVs.
