First published January 18, 2011 doi:10.1152/physiolgenomics.00203.2010.-Long-term ethanol exposure leads to a sexually dimorphic response in both the susceptibility to cardiac pathology (protective effect of the female heart) and the expression of selected myocardial proteins. The purpose of the present study was to use proteomics to examine the effect of chronic alcohol consumption on a broader array of cardiac proteins and how these were affected between the sexes. Male and female rats were maintained for 18 wk on a 40% ethanol-containing diet in which alcohol was provided in drinking water and agar blocks. Differences in the content of specific cardiac proteins in isopycnic centrifugal fractions were determined using mass spectrometry on iTRAQlabeled tryptic fragments. A random effects model of meta-analysis was developed to combine the results from multiple iTRAQ experiments. Analysis of a network of proteins involved in cardiovascular system development and function showed that troponins were oppositely regulated by alcohol exposure in females (upregulated) vs. males (downregulated), and this effect was validated by Western blot analysis. Pathway analysis also revealed that alcohol-consuming males showed increased expression of proteins involved in various steps of oxidative phosphorylation including complexes I, III, IV, and V, whereas females showed no change or decreased content. One implication from these findings is that females may be protected from the toxic effects of alcohol due to their ability to maintain contractile function, maintain efficiency of force generation, and minimize oxidative stress. However, the alcohol-induced insult may lead to increased production of reactive oxygen species and structural abnormalities in male myocardium. iTRAQ; mass spectrometry; meta-analysis; random effects ALCOHOL ABUSE REMAINS ONE of the most common forms of drug abuse among both sexes in the United States. Although the risk of cardiovascular diseases is reduced with moderate alcohol consumption, potential consequences of excessive and chronic alcohol abuse, defined as Ͼ80 g of alcohol per day for longer than 5 yr, include heart dysfunction and heart failure (47). This quantity and frequency of ethanol consumption are sufficient to induce alcoholic heart muscle disease (AHMD) and, in approximately one-third of the cases, contributes to the development of a dilated cardiomyopathy (DCM: 48,50). Abstract. The aim of the present study was to investigate the correlation between serum parathyroid hormone (PTH) levels and coronary artery calcification (CAC) in patients without renal failure, as well as to determine independent risk factors of CAC score (CACS). A total of 157 patients who underwent coronary computed tomography angiographic examination at the 101th Hospital of the People's Liberation Army between December 2013 and February 2015 were retrospectively evaluated. The correlation between PTH levels and CACS was determined using a Pearson correlation analysis. A receiver operating characteristic (ROC) curve was drawn to determine the best cutoff PTH level for prediction of CAC. The independent association between serum PTH levels and CAC was analyzed by using a logistic regression analysis model with the response variable Be binary class. The results revealed that PTH levels in patients in the CAC group were significantly higher than those of patients in the non-calcification group. PTH levels were positively correlated with CACS (r=0.288, P<0.001). The ROC curve suggested that a PTH level of ≥31.05 pg/ml was the best cut-off point for the prediction of CAC, with a sensitivity of 80.88%, specificity of 60.67% and an area under the curve of 0.761. After including predictive factors for CAC (gender, age, smoking status, diabetes, hypertension, hyperlipidemia, body mass index, glomerular filtration rate and calcium, phosphorus, calcium-phosphorus product, magnesium, PTH, total cholesterol, low-density lipoprotein cholesterol, triglyceride, high-density lipoprotein cholesterol and C-reactive protein levels), the odds ratio of the serum PTH levels regarding the prediction of CAC was 1.050 (95% confidence interval, 1.027-1.074; P<0.001). In conclusion, the present study suggested that serum PTH levels are correlated with CAC in patients without renal failure and may thus be used as a reliable predictor of CAC.

Rassegna 19I soggetti con diabete, ipertensione arteriosa, malattie cardiache, obesità, grande anzianità, esposizione a fumo di sigaretta e basso stato sociale hanno maggiori probabilità di essere affetti da IRC. Tali fattori di rischio per la comparsa o la progressione dell'IRC sono in gran parte comuni a quelli per le malattie cardiovascolari. L'IRC è un marcatore di malattia cardiovascolare; alterazioni della funzione renale o dell'esame delle urine possono precedere la comparsa di sintomi di cardiopatia ischemica o l'evidenza strumentale o laboratoristica di una malattia cardiovascolare. Un intervento precoce centrato sui pazienti con IRC aumenta le possibilità di successo della prevenzione e della cura delle malattie cardiovascolari; per tale motivo, la valutazione della funzione renale sta assumendo una crescente importanza, soprattutto in termini di prevenzione secondaria delle complicanze dell'insufficienza renale cronica, potenzialmente letali. Purtroppo risulta, però, scarsa la consapevolezza delle dimensioni del problema e, di conseguenza, dell'importan-L'insufficienza renale cronica (IRC), definita come riduzione del GFR <60 mL/ min/1.73m 2 , è una delle patologie croniche in forte espansione, soprattutto per l'aumento dell'età media della popolazione e dell'incidenza di patologie cardiovascolari e del diabete. la prevalenza della malattia renale cronica è in continuo aumento, anche se di difficile stima, e a ciò concorrono la scarsa sintomaticità e la bassa identificazione negli più precoci.
