Objective: To report the clinical and surgical outcomes of patients with myopic foveoschisis (MF). Methods: Thirteen eyes in ten symptomatic MF patients who underwent pars plana vitrectomy (PPV) and gas tamponade with internal limiting membrane (ILM) peeling were retrospectively identified. Best corrected visual acuity (BCVA) and central foveal thickness (CFT) were assessed preoperatively and six months postoperatively. Complications during and after surgery were also recorded. Results: The median BCVA significantly improved from 20/160 to 20/100 (p = 0.03). In subgroup analysis, eight eyes (61%) demonstrated significant BCVA improvements from 20/80 to 20/50 (p < 0.001), whereas five eyes had similar BCVA with preoperative level after six months of surgery (p=0.32). The mean CFT decreased from 526 microns at baseline to 214 microns at six months of surgery (p < 0.001). Optical coherence tomography scans revealed a complete MF resolution in ten eyes (76%) and a partial MF resolution in three eyes. Peripheral retinal tear was developed in one patient during surgery and one patient had cataract after surgery, those were treated accordingly. Conclusion: PPV with ILM peeling followed by gas tamponade is a safe and effective procedure for the treatment of eyes with myopic foveoschisis. Larger studies with longer follow-ups will further establish the efficacy and safety of this procedure.

Background: Highly myopic eyes differ in morphology from emmetropic eyes, and the correct estimation of the vitreous volume is difficult. To explore an effective method to estimate ocular volume using refractive factors in children.Methods: This is a retrospective study of children with high myopia who visited the Shenzhen Shekou People's Hospital (July-December 2018) before undergoing posterior scleral reinforcement surgery. Data on refractive factors and ocular 3D reconstruction imaging based on high-end CT were collected for linear correlation and linear regression analyses.Results: Ten patients (20 eyes) were included. There are nine males and one female. They were 4 to 12 years of age. The spherical equivalent ranges from +0.25 to -20.00 D. The cylindrical equivalent ranges from -0.50 to -6.25 D. The axial length ranges from 21.78 to 33.90 mm. The corneal curvature (mean) ranges from 42.44 to 46.75. The 3D reconstruction of the CT images shows that the ocular volume ranges from 4.591 to 10.988 ml. The ocular volume of the 20 eyes decreases with the increase of diopter and total curvature, both presenting a linear trend, with the Pearson correlation coefficients being -0.776 (P<0.001) and -0.633 (P=0.003), respectively. The ocular volume of the 20 eyes increases with the increasing axial length, also presenting a linear trend, with the Pearson correlation coefficient being 0.939 (P<0.001).Conclusions: In children, the ocular volume is negatively and linearly correlated with the diopter and curvature, and positively and linearly correlated with the axial length.

For decades there was a general consensus about diagnostic investigations and surgical treatment of symptomatic pathological changes in the vitreomacular interface (VMI). The introduction of SD-OCT imaging helped in the understanding of the pathogenetic processes at this interface and risk factors were defined for the macular traction syndrome, epiretinal membrane (ERM) and macular hole. After approval of ocriplasmin for non-surgical treatment, a new classification based on treatment outcome and new imaging techniques was established. Precise separation of physiological, age-related changes in the VMI and pathological changes was then possible. Clinically relevant aspects in the diagnostic testing and treatment of diseases of the VMI are reported in this literature review.

Abstract: PPV generally resulted in stable or improved VA for eyes with myopic macular retinoschisis. Eyes with concurrent macular hole or foveal retinal detachment often had improved VA, but to a lesser extent than eyes with myopic macular retinoschisis alone.
