Parvovirus B19 in pregnancy

Abstract: Clinicians need to be aware of the risk of adverse outcome of parvovirus B19 infection in pregnancy, and sometimes the long interval between exposure and fetal symptoms. Accurate diagnosis using PCR and weekly ultrasound checks ups with Doppler measurement of middle cerebral artery flow velocity up to 20 weeks postexposure may improve detection of fetal disease. More timely treatment likely results in improved outcome.

“…B19V exposure in early pregnancy may increase the rate of spontaneous abortion. Fetal demise was recognized early and has been extensively reviewed (488)(489)(490)(491)(492)(493)(494)(495)(496)(497)(498). From seroepidemiology, an annual loss of 120 fetuses was calculated for Japan (499), and about twice this number might be extrapolated for the United States.…”

“…This leads to the obvious suggestion that IUT should preferably be done before hydrops develops. In the absence of screening programs for parvovirus susceptibility or infection in pregnant women, the majority is still referred only after the detection of hydrops [26,27,28,29]. …”

“…Maternally administrated IVIG therapy during pregnancy for improving maternal and fetal outcome in severe B19 virus-induced disorders has also been reported in two cases without obvious complications [ 8 , 9 ]. Weekly fetal ultrasound examination including estimation of middle cerebral artery peak systolic velocity (MCA-PSV) for timely detection of possible fetal anemia and hydrops fetalis during a post-exposure period of 12 to 20 weeks was highly recommended [ 10 ].…”
