Patellar Transskeletal Traction for the Treatment of Chronic Patellar Pseudoarthrosis

Abstract: Patellar fractures, which constitute approximately 1% of bone lesions, may lead to severe impairment of the extensor mechanism. When conservative or surgical treatment fails, the patella may develop pseudoarthrosis. Neglect or delayed treatment of this type of injury may lead to significant diastasis between the patellar fragments. There is no consensus regarding the best treatment for such cases. This study is aimed at describing a rare case of patellar pseudoarthrosis in a patient who underwent two-step surg… Show more

“…In 2019, we published the case report of a patient with patellar pseudarthrosis with 9 years of evolution, presenting a distance between the fragments of the patella of 9 cm. 30 We performed patellar trans-skeletal traction, decreasing this distance to 1.2 cm, and performed patellar osteosynthesis in a conventional manner, presenting an excellent functional clinical result with an active range of motion of 0 to 90 .…”

“…3 The proximal retraction of the quadriceps tendon makes the correct distal positioning of the patella difficult. [3][4][5][6] Patellar tendon injury is not an uncommon misdiagnosis at the emergency leading to chronic proximal retraction and, hence, to a challenging clinical scenario sometimes so intense that it may require gradual trans-osseous traction and a two-stage PT reconstruction. 4 Additionally, in the cases of chronic PT rupture, different techniques involving auto or allograft augmentation, reinforcement, and combination of other varying fixation devices were described aiming to appropriately correct the patella height and restore soft tissue integrity.…”
