AimAlthough a thin endometrium has been well recognized as a critical factor in implantation failure, little information is available regarding the molecular mechanisms. The present study investigated these mechanisms by using genome‐wide mRNA expression analysis.MethodsThin and normal endometrial tissue was obtained from a total of six women during the mid‐luteal phase of the menstrual cycle. The transcriptomes were analyzed with a microarray. Differentially expressed genes were classified according to Gene Ontology (GO) terms and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathways.ResultsThe study identified 318 up‐regulated genes and 322 down‐regulated genes in the thin endometrium, compared to the control endometrium. The GO and KEGG pathway analyses indicated that the thin endometrium possessed aberrantly activated immunity and natural killer cell cytotoxicity that was accompanied by an increased number of inflammatory cytokines, such as IFN‐γ. Various genes that were related to metabolism and anti‐oxidative stress were down‐regulated in the thin endometrium.ConclusionImplantation failure in the thin endometrium appears to be associated with an aberrantly activated inflammatory environment and aberrantly decreased response to oxidative stress.

Introduction. The application of modern methods for assessing the morphofunctional state of the endometrium to verify and study the expression of sex steroid hormones, proinflammatory markers and markers of angiogenesis using confocal laser scanning microscopy will allow an objective study of the role of studied markers in the pathogenesis of the endometrial dysfunction. The aim of the study was to evaluate the expression of ER and PR receptors in endometrium in patients with endometrial dysfunction. Material and methods. Endometrial biopsy specimens obtained with the aid of a pile-biopsy or a scraping from the uterine cavity are used to conduct the method of immunofluorescent confocal laser scanning microscopy. It is possible to use both cryostat material and paraffin blocks to provide the immunohistochemical analysis. Monoclonal antibodies to ER (1 : 60, Dako, Denmark) and PR (1 : 50, Dako, Denmark) are used as primary antibodies, antibodies conjugated with fluorochrome Alexa Fluor 647 (1 : 1000, Abcam, England) are used as secondary antibodies). Hoechst 33258 (Sigma, USA) is used for staining of cell nuclei. Results. A method of confocal laser scanning microscopy makes it possible to conduct qualitative and quantitative evaluation of the studied markers in different structures of the endometrium.

Согласно существующим представлениям, все причины безуспешности попыток ЭКО, в которых удается выполнить перенос полученных in vitro эм-брионов, могут быть связаны либо с их неспособно-стью к внутриматочной нидации (эмбриональный фактор), либо со снижением рецептивности эндоме-трия (эндометриальный фактор) [1,2]. Необходи-мость совершенствования эндометриального факто-ра программ ЭКО определяется тем, что при исполь-зовании морфологически хороших эмбрионов, по-лученных при оплодотворении донорских ооцитов в программах ЭКО-ОД, только ненамного повышает частоту успешных имплантаций в сравнении со стандартным ЭКО (где качество переносимых эм-брионов может быть не столь хорошим). Это можно видеть из отчета РАРЧ за 2013 г. (опубликован в 2015 г.), согласно которому частота наступления бе-ременности на перенос эмбрионов (ЧНБ пэ ) в про-грамме ЭКО-ОД превысила лишь на 5,3% аналогич-ный показатель в стандартной программе ЭКО (43,8% против 38,5%) [3].
