Patients over 65 years are assigned lower ECOG PS scores than younger patients, although objectively measured physical activity is no different

Abstract: PA levels were low, but PS scoring reflected relative PA levels and differentiated between patients of PS 0 and 2-3. Chronological age was not predictive of activity levels, but older patients were assigned lower PS scores. Incorporation of objective PA measures may merit further investigation especially in the geriatric oncology setting.

“…56 In another prospective study, 100 treatment-naïve patients with cancer wore an accelerometer before their oncology office visit. 57 A significant inverse association between objectively measured activity and ECOG PS was reported. Accelerometer-assessed activity was able to differentiate patients with clinician-assigned ECOG PS of 0 from those with ECOG PS of 2 to 3 but not from those with ECOG PS of 1.…”

“… 3 5 In a study comparing physician assessments to objective measurements, 80% of patients who were assigned ECOG PS scores of 0 or 1 actually spent >50% of waking hours resting, which corresponds to a PS score of 3. 6 There is a large variation within ECOG PS categories that necessitates more detailed and discriminate assessment of physical function than what is captured.…”
