Payer Status and Increased Distance Traveled for Fracture Care in a Rural State

Abstract: Increasing volume of orthopaedic trauma at a rural level 1 trauma center was associated with increasing travel distance for patients. Specifically, there was a dramatic increase in the volume of patients presenting for the care of simple orthopaedic injuries, these patients traveled greater distances with time and were more likely to beon Medicaid. Patients presenting with emergent injuries were also more likely to be on Medicaid. The appropriate triage of orthopaedic injuries requires a well-designed trauma s… Show more

“…First, because most snakebites are not immediately lethal, a rural snakebite victim may travel to a suburban or urban area to receive medical treatment. Second, patients may seek care from larger, urban academic medical centers believing they offer more advanced clinical capabilities 27–30. A third explanation is that human expansion and snake habitat loss may put humans in traditionally non-rural areas in closer proximity to venomous snakes 31–33.…”

“…Exclusion criteria included pathologic fractures, patients with a healed fracture presenting with infection, patients younger than 18 years, and patients with less than 1 year of follow-up. Distance traveled was measured as a surrogate for barriers to care 15,16 and was calculated as the straight-line distance between the center of the patient's home zip code and the hospital's location.
