Pediatric Invasive Aspergillosis: A Multicenter Retrospective Analysis of 139 Contemporary Cases

Abstract: Our analyses revealed common findings between adult and pediatric invasive aspergillosis. However, one key difference is diagnostic radiologic findings. Unlike adults, children frequently do not manifest cavitation or the air crescent or halo signs, and this can significantly impact diagnosis. Immune reconstitution, rather than specific antifungal therapy, was found to be the best predictor of survival.

“…Notably, almost all patients included in this ana lysis had at least one risk factor. The overall fatality rate of IA in this study was 53%, which is similar to that seen adult patients [11]. Surprisingly, 46% of the children received at least three concomitant antifungal agents for at least 3 days, in most cases a lipid formulation of amphotericin B and voriconazole; however, no superiority of Relatively little is known regarding zygomycosis in children.…”

“…A retrospective study revealed that corticosteroid therapy, neutro penia, immunosuppressive therapy, malignancy and allogeneic HSCT were the most common risk factors for IA in the pediatric age group [11]. Notably, almost all patients included in this ana lysis had at least one risk factor.…”

“…Documented IA with a death rate of 80-90% is difficult to treat [1,2]. In two large series of studies, the most common underlying condition for IA was malignancy (74% and 63%), with the case fatality of 53% [3,4].…”

“…Tomografia komputerowa płuc wysokiej rozdzielczości (high resolution computed tomography -HRCT) jest obecnie ważnym narzędziem diagnostycznym u chorych w neutropenii, realnie przyspieszają-cym ustalenie rozpoznania IA, dodatkowo o znaczeniu prognostycznym [24].
