Pedicle screw fixation combined with intermediate screw at the fracture level for treatment of thoracolumbar fractures

Abstract: Background:Use of a pedicle screw at the level of fracture, also known as an intermediate screw, has been shown to improve clinical results in managing thoracolumbar(TL) fractures, but there is a paucity of powerful evidence to support the claim. The aim of this study was to compare outcomes between pedicle screw fixation combined with intermediate screw at the fracture level and conventional pedicle screw fixation (one level above and one level below the fracture level) for thoracolumbar (TL) fractures.Method… Show more

“…Given that there is no anterior support as long as the fractured segments have not healed, we always attempt to place at least one pedicle screw also in the fractured vertebra as this has shown to increase the construct strength and reduces the risk for later loss of alignment. [ 9 ] We try to place the screws in the fractured vertebra in an oblique direction to provide as much purchase within the pedicle as possible as this is the only stable part in the fractured vertebra [ Figure 2c ].…”

“…Four pedicle screws were inserted into the vertebrae cephalad and caudal to the fractured vertebra. Two polyaxial screws were inserted into both pedicles of the fractured vertebra as intermediate screws [20,21]. The intermediate screw heads were left slightly protruding to act as a push point and to achieve the reduction of kyphosis.…”
