Perceived Roles in Medication Safety: Patients' Perspectives

Abstract: Community-dwelling older adults are vulnerable to medication safety-related harms. Prevention of medication-related harms in the outpatient setting starts with thorough and thoughtful medication reconciliation at each patient encounter.
