Kardiyovasküler hastalık (KVH) en sık ölüm ve morbidite nedenlerindendir. İleri yaş aterosklerozun ilerlemesinde en önemli risk faktörlerinden biridir. Koroner arter hastalığı (KAH) genellikle erkeklerde 45 yaşın ve kadın popülasyonda 55 yaş üstünde görülür. Bununla birlikte, genç popülasyonda miyokard enfarktüsünün (MI) prevalansında önemli bir artış vardır. Bu çalışmanın amacı, kalp şikayetleri ile başvuran ve koroner anjiyografi (KAG) yapılan genç popülasyonda KAH için en yaygın risk faktörlerini tanımlamak ve tedavi stratejileri ile birlikte birincil ve ikincil önleme stratejilerini belirlemektir. Metod: Son 10 yıl içinde kardiyak şikayetlerle gelerek KAG yapılmış genç hastalar geriye dönük olarak tarandı. Çalışmaya toplam 113 olgu alındı. Revaskülarizasyon ihtiyaçlarına göre iki ana gruba ayrıldı. KAG sonrası koroner revaskülarizasyon gerektirmeyen olgular kontrol, (Grup 1); Koroner revaskülarizasyon ihtiyacı olanlar ise hasta grubu (Grup 2) şeklinde sınıflandırıldı. İstatistiksel analiz için SPSS 15.0 for Windows programı kullanıldı. Kategorik değişkenler için tanımlayıcı istatistikler; sayı ve yüzde olarak ifade edildi. Sayısal değişkenler için tanımlayıcı istatistikler; ortalama, standart sapma, minimum, maksimum, medyan şeklinde ifade edildi. İki değişkenli bağımsız değişkenlerin sayısal dağılımları normal dağılım koşulu sağladığında, Student t testi, normal dağılım koşulu sağlanamadığında Mann-Whitney U testi yapıldı. Bağımsız gruplardaki oranlar Ki kare testi ile karşılaştırıldı. Alfa'nın istatistiksel anlamlılık düzeyi p<0,05 olarak kabul edildi. Sonuç: Erkek cinsiyet prevalansı, gruplar arasında kadın cinsiyetten anlamlı olarak daha yüksekti (p=0.002). BKİ'nin ortalama değerinde gruplar arasında istatistiksel olarak anlamlı bir fark yoktu (p=0.105). Sigara ve ortalama sigara sayısı Grup 2'de anlamlı derecede yüksekti (p<0.001; p=0.002). Aim. To study the features of risk profile, coronary artery patterns, and percutaneous coronary intervention in patients aged below 40 years with acute coronary syndrome and stable angina. Methods. 208 patients with coronary artery disease aged below 40 years were examined: 51 (24.5%) patients aged 35 years and younger and 157 (75.5%) aged 3640 years. 98 (47.1%) patients were admitted with acute coronary syndrome; 110 (52.9%) patients with stable angina. In groups of acute coronary syndrome and stable angina, myocardial infarction in past medical history was revealed in 23.5% and 36.4%, respectively. 165 patients underwent percutaneous coronary intervention: 84 (50.9%) with acute coronary syndrome; 81 (40.1%) with stable angina. Results. Patients with stable angina differed by prevalence of myocardial infarction in past medical history, overweight, and family history of coronary artery disease. In group of acute coronary syndrome urban cohort prevailed as well as consumption of energy drinks among patients below 35 years; high prevalence of left ventricular dysfunction. Patients with acute coronary syndrome were characterized by involvement of one and three coronary arteries, and patients with stable angina by pathology of two and three coronary arteries. Involvement of three coronary arteries was equal in both groups. In both groups, anterior interventricular artery was target coronary artery. Patients with stable angina had the same rate of right coronary artery and left circumflex artery involvement. In patients with stable angina, right coronary artery involvement was rarer, and left main coronary artery involvement was two times more frequent than in patients with acute coronary syndrome. The group with acute coronary syndrome was characterized by predominance of discrete lesions and coronary occlusions over diffuse lesions; and the group of stable angina by diffuse lesions, and two-times less frequent coronary occlusions. Conclusion. Among patients with acute coronary syndrome aged below 36 years, revascularization of right coronary artery was predominant, and among patients aged 3640 years with acute coronary syndrome revascularization of left circumflex artery.

Cardiovascular diseases remain a common cause of morbidity and mortality in populations around the world, varying in incidence by age and gender. Every year, more than 7 million people around the world are diagnosed with acute coronary syndrome, which may be the first clinical manifestation of coronary heart disease. In the ranking of mortality causes in the Russian Federation, diseases of the circulatory system occupy a leading place. The purpose of the review was to analyze the clinical characteristics and spectrum of coronary lesions of patients with acute coronary syndrome depending on age and gender. Thus, according to the results of coronary angiography, young patients with myocardial infarction were rarely accompanied by multivessel or ostial lesions of the coronary arteries, in contrast to the elderly patients. It was revealed that multivessel lesions of the coronary arteries more often characterize males, and singlevessel lesions more often characterize females. It is obvious that the prognosis of patients with acute coronary syndrome will largely depend not only on age and gender, but also on the outcome of the syndrome, the quality and scope of diagnostic and treatment procedures. This is especially important since most patients under 60 years of age are exposed to some form of systematic work load in their field of activity. The influence of polypathy, characteristic of modern cardiac patients, can be expressed to varying degrees depending on the gender and age of the patient with acute coronary syndrome. The article presents data on the short-term and long-term prognosis of patients with acute coronary syndrome, taking into account gender and age periods, including depending on the tactics of their management. Determining the clinical characteristics of patients of different ages allows to take them into account for selecting effective and safe therapy, preventing wave-like atherosclerosis as the main cause of coronary heart disease, thereby improving prognosis and survival.
