Performance de l’IRM bas champ dans la rupture du Ligament Croisé Antérieur du genou.

Abstract

Objectives: Magnetic Resonance Imaging (MRI) remains the gold-standard examination for the diagnosis of Anterior Cruciate Ligament (ACL) complete rupture. Arthroscopy is an extremely sensitive and specific procedure for the evaluation of ACL, but it is invasive. Just few authors compared the performance of low field MRIs with the results of arthroscopy. We therefore proposed to compare the low-field MRI performance in the ACL complete ruptures with the results of the arthroscopy. Materials and Methods: This is a retrospective study of 30 patients with complete or partial ACL ruptures who underwent an MRI and an arthroscopy within one month after the MRI from July 18, 2016 to July 19, 2017. 15 patients underwent a high-field MRI, and 15 patients a low-field MRI. The results of arthroscopy were considered the gold standard. We calculated sensitivity, specificity, positive predictive value, and negative predictive value for low and high-field MRI. Results: The population consisted of 30 patients, 25 men and 5 women, average age 27.2-year-old (17-41). Low-field MRI (0.3T) revealed 11 out of 15 (73.3%) complete ruptures, compared to 10 out of 15 (66.6%) confirmed by the arthroscopy, 4 out of 15 (26.6%) partial ruptures, compared to 5 out of 15 (33.3%) confirmed by the arthroscopy. The sensitivity of MRI 0.3 T was 100%, the specificity at 80%, the VPP at 90.91%, the VPN at 100% (P value: 0 .001). The others 15 patients (15/30) were explored with a high-field MRI (1.5T) and it revealed 12 out of 15 (80%) complete ruptures, while arthroscopy showed 13 out of 15 (86.6%) complete ruptures, and 3 out of 15 (20%) partial ruptures versus 2 out of 15 (13.3%) demonstrated by arthroscopy. The sensitivity of MRI 1.5 T for the detection of complete rupture was 92.31%, the specificity 100%, the VPP 100%, the VPN at 66.67%, (P - value - 0 .001). Conclusion: The diagnostic performance of low-field MRI is comparable to high-field MRI for identification of LCA complete rupture and it can be added to other known low-field MRI benefits. RÉSUMÉ Objectifs : L’imagerie par résonance magnétique (IRM) reste à ce jour l’examen de référence pour le diagnostic de rupture du ligament croisé antérieur (LCA) du genou. L’arthroscopie est une procédure très sensible et spécifique, mais invasive. Nous nous sommes proposés de comparer les données de l’IRM bas champ dans la rupture complète du LCA aux résultats des arthroscopies. Matériels et méthodes : Il s’agissait d’une étude rétrospective, effectuée sur 30 patients porteurs d’une rupture du LCA complète ou partielle, ayant bénéficié d’une IRM et d’une arthroscopie. 15 patients avaient bénéficié d’une exploration par IRM haut champ, et 15 patients par bas champ. La durée de l’étude était de 12 mois, du 18 Juillet 2016 au 19 juillet 2017. Les résultats de l’arthroscopie ont été considérés comme le gold standard. Nous avons calculé la sensibilité, la spécificité, la valeur prédictive positive et la valeur prédictive négative de l’IRM bas champ. Ensuite la performance diagnostique a été évaluée aussi pour l’IRM haut champ. Résultats : 30 patients, 25 hommes et 5 femmes, âge moyen 27,2 ans (17-41 ans). Parmi les 15 patients qui ont reçu un examen dans une IRM bas champ (0.3T), on a observé 11 (73,3%) ruptures complètes, contre 10 (66,6%) confirmés par l’arthroscopie, 4 (26,6%) ruptures partielles, contre 5 (33,3%) démontrées à l’arthroscopie. La sensibilité de l’IRM 0.3 T pour la détection de la rupture complète s’élevait à 100%, la spécificité à 80%, la VPP à 90.91%, la VPN à 100% (P - value = 0 .001). Les autres 15 patients (15/30) ont été explorés avec une IRM haut champ (1.5T) et on a alors observé 12 (80%) ruptures complètes, alors que l’arthroscopie a démontré 13 (86,6%), et 3 (20%) ruptures partielles contre 2 (13,3%) démontrées par l’arthroscopie. La sensibilité de l’IRM 1.5 T pour la détection de la rupture complète s’élevait à 92.31%, la spécificité à 100%, la VPP à 100%, la VPN à 66.67%, (P - value = 0 .001). Conclusion : Une performance diagnostique de l’IRM à bas champ tout à fait comparable à l’IRM à haut champ pour l’identification des ruptures complètes de LCA peut être ajoutée aux avantages des IRM bas champ déjà connus.
