The kidney plays a vital role in the body's defense against potentially toxic xenobiotics and metabolic waste products through elimination pathways. In particular, secretory transporters in the proximal tubule are major determinants of the disposition of xenobiotics, including many prescription drugs. In the past decade, considerable progress has been made in understanding the impact of renal transporters on the disposition of many clinically used drugs. In addition, renal transporters have been implicated as sites for numerous clinically important drug-drug interactions. This review begins with a description of renal drug handling and presents relevant equations for the calculation of renal clearance, including filtration and secretory clearance. In addition, data on the localization, expression, substrates, and inhibitors of renal drug transporters are tabulated. The recent US Food and Drug Administration drug-drug interaction draft guidance as it pertains to the study of renal drug transporters is presented. Renal drug elimination in special populations and transporter splicing variants are also described.

ObjectiveThe present study aimed to establish reference intervals for serum cystatin C (Scys-C) stratified by stages of chronic kidney disease, explore factors influencing Scys-C and compare the performance of Scys-C with serum creatinine (Scr) in the young and elderly.MethodsA total of 800 participants, 516 young (<60 years) and 284 old (≥60 years) subjects were included in this study. Scys-C and Scr were assayed by the partical-enhanced immunoturbidimetry method and enzymatic method respectively. 95% reference interval was adopted to evaluate reference intervals. Influencing factors were characterized by multivariate linear regression analysis. Relationship between reference glomerular filtration rate (rGFR) and Scys-C or Scr was determined by correlation coefficient.ResultsReference intervals for Scys-C were calculated to be 0.71–1.38 mg/L, 0.83–1.67 mg/L, 1.02–2.61 mg/L, 1.32–4.48 mg/L, 1.95–6.11 mg/L in the aged in CKD G1, G2, G3a, G3b and G4-5 stages, respectively. Body mass index(BMI), nephritis, kidney neoplasm and hypertension were demonstrated as factors affecting Scys-C in the elderly while gender, nephritis and kidney neoplasm were clarified as influencing factors in the young group. Scr levels were affected by more factors, such as body surface area and hematological disease. Correlation coefficient between rGFR and Scys-C or Scr showed that serum Scys-C was superior to Scr, especially in the subjects with mildly decreased renal function (−0.593 vs. −0.520).ConclusionsFactors other than renal function influenced Scys-C when applying to evaluate glomerular filtration rate (GFR), such as BMI, nephritis, kidney neoplasm and hypertension, and Scys-C had higher correlation with GFR than Scr in the elderly.

Bevezetés: Krónikus vesebetegségben elengedhetetlen a glomerulusfi ltrációs ráta ismerete, mivel annak mértéke határozza meg a stádiumokat, és iránymutató a kezelés szempontjából. Célkitűzés: A szerzők két, a standardizált kreatininen, valamint öt immunturbidimetriás cisztatin-C-meghatározáson alapuló becsült glomerulusfi ltrációs ráta egyenletét vizsgálták. Módszer: Az analitok és az egyenletek eloszlását, kapcsolataikat, a becsült glomerulusfi ltrációs ráták közötti különbségeket és stádiumbesorolásaikat tanulmányozták. Eredmények: A cisztatin-C-képletek több beteget soroltak az 1-es stádiumba, míg a kreatinines képletek a 2-es, 3-as és 4-esbe. Az 5. stádiumba a Grubb1-, Grubb2-és Larsson-képletek több, míg a Tan-és Sjöström-képletek kevesebb beteget soroltak, mint a kreatinines képletek. Legjobban a Grubb1-és Grubb2-egyenletek stádiumbesorolásai hasonlítottak egymásra. A 3-5. stádiumok legritkábban a Sjöström-képlet esetében fordultak elő. Aims: Two standardized creatinine based estimated glomerular fi ltration rate equations and fi ve equations based on the immunoturbidimetric determination of cystatin C were compared. Methods: The distribution of the analytes and the equations, their relations, as well as the differences among the estimated glomerular fi ltration rates and their chronic kidney disease stages assignments were studied. Results: The equations based on cystatin C classifi ed more patient into stage 1, while the creatinine based ones more into stages 2, 3 and 4. The equations published as Grubb1, Grubb2 and Larsson classifi ed more patients while the equations created by Tan and Sjöström classifi ed fewer into stage 5 compared to the creatinine based equations. The equations of Grubb1 and Grubb2 resulted in the most similar stage assignment. The occurence of stages between 3 and 5 was the lowest using the equation of Sjöström. Conclusions: The different equations for the estimation of glomerular fi ltration rate modify signifi cantly the chronic kidney disease stage assignment which may have an infl uence on the treatment and outcome measures of the patients.
