Peritoneal Resting with Heparinized Lavage Reverses Peritoneal Type I Membrane Failure. A Comparative Study of the Resting Effects on Normal Membranes

Abstract: Peritoneal rest induces functional changes in patients with UFF but not in those with no functional abnormalities. This demonstrates that PR works only when abnormal but reversible functional conditions are present. However, the effect is highly dependent on how early PR is applied.

“…It may also be an excellent resource for pediatric PD patients, who need high ultrafiltration and also nutrition that could be provided by SCPD with glucose and amino acid solutions (18,19). Also, the low glucose concentrations achieved by SCPD would preserve peritoneal function in the long term, perhaps even improving it in the same way as peritoneal rest (43), but without the need of discontinuing PD.…”

“…The question of whether peritoneal rest facilitates recovery of the membrane in patients who have developed UFF has been explored in a recent single-center retrospective study (32). The results suggested that it may have a role in early UFF but not after it has become established.…”

“…The results suggested that it may have a role in early UFF but not after it has become established. De Sousa et al (32) hypothesized that peritoneal rest may interrupt the process of epidermal-mesenchymal transformation, which is considered to be important in the development of peritoneal membrane change, possibly allowing for its reversal.…”

“…Furthermore, reports have indicated that the long-term use of icodextrin dialysis fluid (limited to cases of once per day) has gradually resulted in the loss of ultrafiltration [75], so attention must be taken. The implementation of peritoneal resting (d) is not necessarily efficacious for all patients, either; however, there have been many reports on this method, with the oldest going back to the 1990s [76][77][78][79][80][81].…”
