Permanent retinal damage following massive dapsone overdose.

Abstract: SUMMARY A massive dose of 7 5 g of 4,4'-diamino, diphenyl sulphone (dapsone) taken as a suicide attempt in a patient on long-term therapy for tuberculoid leprosy resulted in permanent bilateral retinal necrosis, a previously unreported side effect of this drug. The patient developed a severe haemolytic anaemia, methaemoglobinaemia, and acute renal failure requiring peritoneal dialysis. It is proposed that the retinal damage was due to a combination of severe hypoxaemia and the physical effects of red cell frag… Show more

“…Recovery might occur; however, this may take many months to several years. Tragic cases of patients who had ingested dapsone with suicidal intention confirm the ability of the drug to damage the peripheral nervous system, including the optic nerve resulting in blindness [ 77 , 91 ]. In any case, if any signs of neuropathy show, therapy with dapsone should be immediately stopped (Figs.…”

“…NIRS, therefore, has the advantages of offering a noninvasive, continuous, real-time measurement of brain oxygenation without any known adverse effects. NIRS has been used to monitor cerebral oxygenation during cardiopulmonary bypass and carotid endarterectomy [12,13]. Here we report its use to monitor changes in cerebral oxygenation which occur during hypothermic circulatory arrest and then after the introduction of retrograde cerebral perfusion.…”

“…Blurred vision has been reported and may occur after a period of normal vision. This necessitates careful ophthalmic assessment as the retinal damage may become permanent [13]. Steroids may have a role in this situation.…”
