Persistent acantholytic dermatosis. A variant of transient acantholytic dermatosis (Grover disease)

Abstract: A patient with unusual manifestations of transient acantholytic dermatosis (Grover disease) has lesions that have persisted for more than three years. Biopsy specimen showed histologic changes previously unreported in Grover disease. In order to emphasize that this condition is not always transient, we have titled this entity "persistent acantholytic dermatosis."

“…The latter is probably a variant of the former (Chalet et al 1977). Histopathologically the present case shows a pemphigus vulgaris-like pattern (Feinstein et al 1971, Simon et al 1976, Chalet et al 1977 with some spongiotie changes (Simon et al 1976, Chalet et al 1977) of transient acantholytic dermatosis. Also, ultrastructurally this case mimics pemphigus vulgaris, as mentioned previously.…”

“…Acantholysis may be observed in several skin or mucous membrane diseases, such as pemphigus, Darier disease, benign chronic familial pemphigus (Hailey-Hailey disease), transient acantholytic dermatosis (Grover 1970, Feinstein et al 1971, Simon et al 1976, Waisman et al 1976, Chalet et al 1977, benign papular acantholytic dermatosis (Heaphy et al 1976), focal acantholytic dyskeratosis (Ackerman 1972), viral diseases and tumors.…”

“…Acantholytic changes may be seen in numerous and heterogeneous clinical conditions: (i) genodermatoses (Hailey-Hailey disease, Darier disease); (ii) infections; (iii) pemphigus; (iv) several malignant or premalignant conditions (acantholytic type of solar keratosis, in which one observes immediately above the atypical cells composing the basal cell layer clefts or lacunae similar to those seen in Darier disease [8]or in Paget disease [9]); (v) Grover disease [10, 11, 12]. …”

“…They used the title of benign papular acantholytic dermatosis and noted that the lesions were initiated or exacerbated by stinlight in five of seven cases. In one subsequent patient the eruption lasted over 5 years (Simon, Bloom & Ackerman, 1976) and histologically showed foci of spongiosis in addition to acantholysis, dyskeratosis and suprabasal clefting. Fawcett & Miller (1983) have collected a series of cases of persistent acantholytic dermatosis seen at London hospitals during the period t969 to 1982.…”
