Persistent Helicobacter pylori Infection: An Insight to the Limitations of Current Clinical Practice

Abstract: The discovery of the pathological role of Helicobacter pylori in various disease states, such as peptic ulcer disease (PUD) and Mucosa Associated Lymphoid Tissue (MALT) lymphoma, was ground-breaking in the field of gastroenterology. Given the potentially dire clinical implications of chronic H. pylori infection, it is important to achieve complete eradication. More importantly, the rising prevalence of H. pylori antimicrobial resistance, similar to other pathogens world-wide, is of particular concern. Despite … Show more

“…Amoxicillin resistance in H. pylori is one of the greatest concerns of clinicians because anti- H. pylori regimens often consist of amoxicillin in addition to other antibiotics and proton pump inhibitors [ 20 , 21 ]. Recent studies have suggested that amoxicillin resistance in H. pylori results from alterations in PBP1A [ 12 , 13 , 15 , 22 ].…”

“… 32 , 33 , 34 The importance of this stimulation in vivo has been clearly demonstrated by the induction of remissions in gastric MALT lymphomas with antibiotic treatment to eradicate Helicobacter pylori. 35 , 36 A role for antigenic stimulation by Chlamydia psittaci and Borrelia burgdorferi has been proposed for some cases of ocular adnexal and cutaneous MALT lymphomas, respectively. 34 , 37 , 38 A similar role has been proposed for Campylobacter infection in patients with heavy chain disease.…”
