Transcatheter aortic valve replacement (TAVR) has been introduced as an alternative to surgical aortic valve replacement for patients with severe aortic valve disease and is expanding into a universal treatment. However, complications after TAVR can have devastating consequences for patients and must be predicted. By designing a TAVR medical twin architecture based on real-world data (RWD), we can minimize complications and achieve optimal clinical outcomes through analysis and simulation results in a virtual environment that can predict complications. The simulation phase utilizes machine learning algorithms for complication prediction to predict patients with conduction abnormalities, a complication of TAVR, and provides the prediction results through a web-based monitoring system. We also conduct research to identify factors that influence complications, so that complication prediction in a virtualized environment on a medical twin architecture can serve as a guide for personalized care design for patients undergoing TAVR.

Digital Twin (DT) technology has experienced substantial advancements and extensive adoption across various industries, aiming to enhance operational efficiency and effectiveness. Defined as virtual replicas of physical objects, systems, or processes, Digital Twins enable real-time simulation, monitoring, and analysis of real-world behavior. This comprehensive review delves into the evolution of DT technology, tracing its journey from conceptual origins to contemporary technological implementations. The review provides detailed definitions, a classification of different types of Digital Twins, and a comparative analysis of their architectures. Furthermore, it investigates the application of DT technology in diverse sectors, with a particular emphasis on medicine and manufacturing, exemplified by use cases such as personalized medicine. Moreover, the review highlights emerging trends and future directions in DT technology, underscoring the transformative potential of integrating artificial intelligence and machine learning to augment DT capabilities. This analysis not only elucidates the current state of DT technology but also anticipates its future trajectory and impact across multiple domains.

Proliferative vitreoretinopathy (PVR) is a pathological process characterized by the formation of fibrotic membranes that contract and lead to recurrent retinal detachment. Pars plana vitrectomy (PPV) is the primary treatment, but recurrence rates remain high, as surgery does not address the underlying molecular mechanisms driving fibrosis. Despite several proposed pharmacological interventions, no approved therapies exist, partly due to challenges in conducting preclinical and in vivo studies for ethical and safety reasons. This review explores the potential of computational models and Digital Twins, which are increasingly gaining attention in medicine. These tools could enable the development of progressively complex PVR models, from basic simulations to patient-specific Digital Twins. Nintedanib, a tyrosine kinase inhibitor targeting PDGFR, VEGFR, and FGFR, is presented as a prototype for computational models to simulate its effects on fibrotic pathways in virtual patient cohorts. Although still in its early stages, the integration of computational models and Digital Twins offers promising avenues for improving PVR management through more personalized therapeutic strategies.
