Phenotypic approach to the Management of chronic prostatitis/chronic pelvic pain syndrome

Abstract: There is no one unifying etiological mechanism or specific curative therapy for chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS). However, there is sufficient evidence to suggest that each of the proposed mechanisms may be important in some patients, and that many of our evaluated treatments do in fact work in subgroups of patients. We hypothesize that CP/CPPS patients are not a homogenous group suffering from a single disease entity. Rather, CP/CPPS patients are actually unique individuals with diff… Show more

“…In 2009, Nickel and Shoskes reported that CP/CPPS should be approached and treated based on its phenotype [33]. And Volkan et al reported that complex treatment was as safe as monotherapy [34] and Nickel et al explained the concept of UPOINT in detail in his CP/CPPS review thesis [35].…”

“…Diagnostic Algorithm (see figure 1) nickel clinically defined presenting phenotypes (UPOINT) have been proposed 58 and the early results of such a strategy look promising. 59 Based on the fact that monotherapies provide (at best) modest efficacy, a multimodal approach using specific clinical phenotypes to choose therapies is considered an optional recommendation.…”
