Physiologic Evidence for a Dual A-V Transmission System

Abstract: A study of the transmission of early premature contractions between atria and ventricles and in the retrograde direction in the dog heart suggests the existence of two parallel A-V conduction pathways communicating with each other over one or more branches. The evidence is based on the excessive delay of very early premature responses in traversing the node, suggesting that a slowly conducting pathway recovers earlier than the normal "fast" pathway; on the echoing back to the chamber of origin of early prematu… Show more

“…It is now possible to extend our observations in man by recording extracellular action potentials from several specialized areas along the A-V conduction system." 1 The results of the present study relate closely to the reports of Moe and co-workers,3 Hoffman and associates,2 and Moore,4 despite the fact that obvious differences in methodology exist between this clinical report and their animal experiments. Among these differences is the fact that in the aforementioned studies the basic driving rate of the atrium was controlled while in this study premature atrial stimulation was performed during sinus rhythm.…”

“…The reason for this finding in our mouse model remains to be clarified. Faster AV-nodal conduction properties and unchanged or even elevated AV-nodal refractory period are not a necessarily contradictory finding, but a common pathophysiological phenomenon in arrhythmias involving fast-conducting AV-nodal pathways (27,28).…”

“…1940 sonrasında reentry mekanizmasının atriyoventrilüer (AV) nodda yerleştiği tespit edildi [6]. İlerleyen yıllarda yapılan hayvan çalışmaları sayesinde AV nodda ikili yolak olduğu ortaya konuldu [7,8]. İnsanlarda yapılan elektrofiyolojik çalışmalarda, oluşturulan bazı atriyal erken vuruların His demetini depolarize etmeden, AV nod refrakter durumda iken sürekli taşikardilere neden olduğu tespit edildi [9].…”
