Plasmablastic Extramedullary Plasmacytoma Associated with Epstein-Barr Virus Arising in an Immunocompetent Patient with Multiple Myeloma

Abstract: We encountered a case of plasmablastic extramedullary plasmacytoma with multiple myeloma. Histological findings revealed that the extramedullary plasmacytoma of this patient was of the plasmablastic type, which was positive for λ-stain and EBV-encoded RNA. In contrast, bone marrow aspiration demonstrated a common-type multiple myeloma, which was positive for λ-stain and negative for EBV-encoded RNA. This was a rare case of plasmablastic extramedullary plasmacytoma associated with Epstein-Barr virus arising in … Show more

“…The authors argue that many such cases may have been misdiagnosed as plasmablastic B lymphomas. The simultaneous presence of EBV negative MM and EBV positive EP in this patient is intriguing . The authors suggest that EBV positive EP cells indeed were derived from the existing MM cells and that the virus may have been responsible for the observed plasmablastic features.…”

“…The association of EBV with this tumor is rare . EBV associated extramedullary plasmacytoma (EP) has been reported in a patient who had EBV negative MM . The authors suggested that the EBV positive plasmacytoma arose from MM cells .…”

“…Chronic irritation from inhaled irritants and viral pathogenesis have been discussed [16][17][18]. To detect recurrence and progression to multiple myeloma, which occurs in 10-30 % of cases, radiological follow-up and electrophoresis assessment are required.…”
