Platelet Hyporeactivity in Very Low Birth Weight Neonates

Abstract: SummaryVery few studies have examined platelet function in very low birth weight (VLBW) preterm neonates, because of the relatively large volumes of blood required. In this study, platelet function in clinically stable VLBW neonates was examined by whole blood flow cytometry, which requires only 5 |jl1 of whole blood per assay. The following monoclonal antibodies were used: S12 (P-selectin-specific, reflecting a granule secretion), PAC1 (directed against the fibrinogen binding site exposed on the GPIIb-IIIa co… Show more

“…In the presence of signifi cant bleeding, immediate reversal using fresh frozen plasma (FFP) or prothrombin complex concentrates or recombinant factor VIIa may be required. 182,183 This hyporeactivity of neonatal platelets is the result of a defect intrinsic to neonatal platelets. 182,183 Paradoxically, the bleeding time is short in newborns because of increased RBC size, high hematocrit levels, and increased levels of multimeric forms of von Willebrand factor.…”

“…10,11 Only minuscule volumes (~5 µL) of blood are required 1,8 making whole blood flow cytometry particularly advantageous for neonatal studies. 12 The platelets of patients with profound thrombocytopenia can also be accurately analyzed. Finally, flow cytometric evaluation of platelets is easily adaptable to animal studies provided the antibody reagents are available.…”

“…Surfactant may contribute to the resolution of this process by improving lung compliance, thus lowering pulmonary vascular resistance. There is no dispute that surfactant therapy improves the oxygenation index in mechanically ventilated patients [5], but its association with PHEM is still unclear [6, 7]. …”
