Plexiform Neurofibroma of the Wrist: Imaging Features and When to Suspect Malignancy

Abstract: Plexiform neurofibromas are essentially pathognomonic for neurofibromatosis type 1 (NF1), occurring when there is diffuse involvement along a nerve segment and its branches. Transformation into a malignant peripheral nerve sheath tumour (MPNST) is a major cause of mortality in NF1 patients. These tumours are highly aggressive and particularly difficult to diagnose in NF1 patients due to the clinical overlap between benign and malignant lesions. We present a case of a plexiform neurofibroma and discuss the typi… Show more

“…Differential diagnosis is essential due to the potential malignant risk of PN, whose clinical diagnosis may be made according to cafe-au-lait spots or axillary freckling[ 20 ]. Radiologically, PN demonstrates T1-hypointense and heterogeneously T2-hyperintense signals or typical target sign with a central fibrocollagenous and peripheral myxoid stroma component[ 21 ]. Usually, encapsulated margins are not shown.…”

“…Neurofibromas have been reported in the thumb [6], palm [7], index finger [8], median nerve [9,10], fifth finger [11], and wrist [12,13]. Interestingly, six cases have been reported for the ulnar nerve, although they were treated with different approaches [14][15][16][17][18][19].…”

“…Marchetti et al [17] opted for a radiosurgical treatment in a case of a small ulnar neurofibroma case, achieving satisfactory clinic results. Gosein [18] reported a neurofibroma bigger than usual (11 cm) in the ulnar nerve that was not completely removed. Although the patient showed no postoperative motor deficits, he reported tingling along the ulnar nerve distribution.…”

“…La exploración con Doppler color puede mostrar signos de vascularización en algunas lesiones. La variante plexiforme se caracteriza por presentar múltiples tractos hipoecogénicos hipovasculares siguiendo una rama nerviosa y los difusos por ser lesiones mal definidas hipo/hiperecogénicas con posibilidad de presentar señal Doppler positiva (17 …”
