Portuguese Consensus on Diagnosis, Treatment, and Management of Anemia in Pediatric Inflammatory Bowel Disease

Abstract: Anemia is a common extraintestinal manifestation of inflammatory bowel disease (IBD), both in pediatric and in adult patients. Iron deficiency is the main cause of anemia in patients with IBD. Anemia is a clinically relevant comorbidity, with impact on patients' quality of life and it should be timely diagnosed and adequately treated. Currently, an active treatment approach is the recommended strategy, with evidence showing efficacy and safety of intravenous iron formulations. However, evidence in pediatric ag… Show more

“…Following the concept of multifactorial development of thrombosis ( 52 ), ID or IDA add to established risk factors as inflammation, steroid therapy, protein loss or dehydration ( 53 , 54 ) further increasing thrombosis risk in children with IBD. These data support trials investigating the impact iron replacement in children with IBD has on the incidence of thromboembolic events ( 4 , 55 ).…”

“…Oral iron therapy is the standard, easily administered, and cheap treatment for the management of pediatric IBD-associated IDA. 2 4 However, it has several limitations including gastrointestinal side effects, poor compliance, and impaired iron absorption due to inflammation. 17 In addition, animal studies proposed that oral elemental iron may catalyze reactions that generate oxygen-free radicals that can exacerbate inflammation and worsen the symptoms of IBD.…”

“…Nevertheless, there is mixed enthusiasm about the use of IV iron in children[ 16 ]. This reluctance likely arises from concerns about adverse reactions associated with IV iron administration and the lack of published data on the clinical efficacy and safety of newer IV iron formulations in this patient population[ 8 ].…”
