Positive correlation between postoperative tumor recurrence and changes in circulating tumor cell counts in pulmonary venous blood (pvCTC) during surgical manipulation in non-small cell lung cancer

Abstract: Increasing pvCTC count during surgical manipulation was significantly correlated with postoperative distant metastasis in completely resected NSCLC patients. Significant shorter TDM was observed in patient with high ΔpvCTC group.

“…The link between CTC detection in lung cancer and the surgery technique was assessed by few authors. Particularly, it was shown that surgical manipulation significantly increased CTC number in the pulmonary vein and that this was associated with lymphatic invasion and a significant reduction of PFS and OS (Hashimoto et al ., , ). A recent work suggested that intraoperative manipulation contributes to the hematogenous dissemination of tumorigenic CTCs and circulating tumor micro‐emboli (Table ) (Lv et al ., ).…”

“…Hashimoto et al reported that patients with an increased number of CTCs after surgery are more susceptible to distant metastasis. 14 Some research suggests that CTC count in the PV is related to clinical and histological factors. Reddy et al reported that the number of CTCs in the PV was associated with pathological tumor size.…”

“…Furthermore, they demonstrated that the PA-first procedure was an independent risk factor for the increase of CTCs during surgery in multivariate analysis. Hashimoto et al (24) reported that increasing CTCs in the PV during surgical manipulation was significantly correlated with postoperative distant metastasis in completely resected NSCLC patients, although neither recurrence-free survival nor overall survival was significantly correlated with the increase in CTCs in PV. Furthermore, Song et al (25) conducted a prospective randomized study related to the invasiveness and adhesiveness of malignant tumor cells and suggested that ligation of the PV should be performed first during lobectomy.…”
