Postcataract Endophthalmitis Caused byEnterococcus faecalis

Abstract: Although all E. faecalis isolates were susceptible to vancomycin, endophthalmitis caused by E. faecalis was usually associated with poor visual prognosis.

“…The conjunctival bacteria isolated in each of the patients were prospectively recorded in the laboratory database, which also contains the medical record numbers and demographic data of the patients, and the origin and collecting date of the samples among other data. Bacteria were grouped as in order to obtain a number of samples as big as possible for carrying out statistical comparisons, and paying attention to their above described pathogenic power in PE cases [17][18][19][20][21][22][23][24][25] (S. aureus, Streptococcus species, Enterococci, Bacillus spp, and most Gram-negative rods).…”

“…The devastating effect of these bacteria was also shown after the EVS in several studies. 4,[20][21][22][23][24][25] Taking into account the role of the conjunctival flora as the main source of bacterial contamination during the intraocular surgery, 26 what is remarkable is the low prevalence of these pathogenic bacteria on the conjunctiva of patients operated on for cataract in all the published studies. [27][28][29][30] However, very little is known about whether the clinical characteristics of this small number of patients with pathogenic bacteria colonising their conjunctivas are different.…”

“…Although E. faecalis is rarely a causative agent of clinical endophthalmitis, it can rapidly induce postoperative endophthalmitis, often within 2-4 days, and can cause substantial vision loss upon infection. [35][36][37] Indeed, patients with E. faecalis-related endophthalmitis had the worst visual outcome in the Endophthalmitis Vitrectomy Study. 38 We found that the IC injection of LVFX significantly reduced inflammation scores and bacterial counts in the vitreous humor and maintained retinal function in our aphakic rabbit model, compared with LVFX ED, MFLX ED, and IC saline treatment.…”

“…13 It can rapidly induce postoperative endophthalmitis, often within 2-4 days, 14 and can cause substantial vision loss upon infection. 13,15,16 Notably, patients with E. faecalis-related endophthalmitis had the worst visual outcome in the Endophthalmitis Vitrectomy Study. 10 Several studies have since examined the contribution of enterococcal virulence factors to the pathogenesis of E. faecalis endophthalmitis.…”
