BackgroundOral squamous cell carcinoma (OSCC) is a widespread disease with only 50%–60% 5‐year survival. Individuals with potentially malignant precursor lesions are at high risk.MethodsSurvival could be increased by effective, affordable, and simple screening methods, along with a shift from incisional tissue biopsies to non‐invasive brush biopsies for cytology diagnosis, which are easy to perform in primary care. Along with the explainable, fast, and objective artificial intelligence characterisation of cells through deep learning, an easy‐to‐use, rapid, and cost‐effective methodology for finding high‐risk lesions is achievable. The collection of cytology samples offers the further opportunity of explorative genomic analysis.ResultsOur prospective multicentre study of patients with leukoplakia yields a vast number of oral keratinocytes. In addition to cytopathological analysis, whole‐slide imaging and the training of deep neural networks, samples are analysed according to a single‐cell RNA sequencing protocol, enabling mapping of the entire keratinocyte transcriptome. Mapping the changes in the genetic profile, based on mRNA expression, facilitates the identification of biomarkers that predict cancer transformation.ConclusionThis position paper highlights non‐invasive methods for identifying patients with oral mucosal lesions at risk of malignant transformation. Reliable non‐invasive methods for screening at‐risk individuals bring the early diagnosis of OSCC within reach. The use of biomarkers to decide on a targeted therapy is most likely to improve the outcome. With the large‐scale collection of samples following patients over time, combined with genomic analysis and modern machine‐learning‐based approaches for finding patterns in data, this path holds great promise.

ObjectivesThis study assessed the validity of nomograms for predicting malignant transformation (MT) among patients with oral leukoplakia (OL) and oral lichen planus (OLP).Materials and MethodsTwo nomograms were identified following a systematic search. Variables to interrogate both nomograms were obtained for a retrospective OL/OLP cohort. Then, the nomograms were applied to estimate MT probabilities twice and their average was used to calculate the discriminatory performance, calibration, and potential net benefit of the models. Subgroup analyses were performed for patients with OL, OLP, and oral epithelial dysplasia.ResultsPredicted probabilities were mostly significantly higher among OL/OLP patients who developed MT compared to those who did not (p = <0.001–0.034). AUC values and Brier scores of the nomograms were 0.644–0.844 and 0.040–0.088 among OL patients and 0.580–0.743 and 0.008–0.072 among OLP patients. Decision curve analysis suggested that the nomograms had some net benefit for risk stratification. However, the models did not best binary dysplasia grading in discriminatory validity and net benefit among patients with OL and oral epithelial dysplasia.ConclusionNomograms for predicting MT may have satisfactory validity among patients with OL than OLP, but they do not outperform binary dysplasia grading in risk stratification of OL.

Introducción: Es necesaria una adecuada interacción multidisciplinaria entre médicos y profesionales de la salud bucodental para disminuir las repercusiones de la enfermedad periodontal. Objetivo: Determinar si aspartato aminotransferasa salival (AST) es mejor biomolécula diagnóstica de periodontitis comparada con AST sérica e Interleucina- 6 (IL-6) y factor de necrosis tumoral-alfa (TNF-α) salivales y séricas. Material y Método: Estudio de metodología mixta, compuesta por dos subestudios, el primero, experimental, comparativo, transversal, retrospectivo para determinar la capacidad diagnóstica de AST, IL-6 y TNF-α salivales y séricas en sujetos sin diagnóstico de periodontitis que acuden al servicio de medicina general y el segundo, experimental, comparativo, longitudinal, para confirmar la capacidad diagnóstica de AST salival (ASTsa), antes y después de tratamiento periodontal en sujetos que acuden al servicio de salud bucodental. El diagnóstico de periodontitis se determinó mediante el índice de Extensión y Severidad (IES). Resultados: Solo los niveles de ASTsa fueron diferentes entre sujetos sin y con periodontitis (p < 0,0005), los cuales correlacionaron con la extensión (r = 0.410, p < 0.0005) y severidad (r = 0.428, p < 0.0005) del IES, y presentaron un punto de corte de 16.0 UI / mL con alta capacidad discriminativa (Área bajo la curva = 0.936, IC 95 %, 0.864 - 1,000; p < 0,001). Además, posterior a tratamiento periodontal los niveles de ASTsa disminuyeron (116.9 ± 76.2. U/L vs 58.8 ± 47.0 U/L, p < 0.001). Conclusión: ASTsa es la mejor biomolécula diagnóstica de periodontitis que podría utilizarse como puente de interacción multidisciplinaria.
