Practice advisory: The utility of EEG theta/beta power ratio in ADHD diagnosis

Abstract: Objective: To evaluate the evidence for EEG theta/beta power ratio for diagnosing, or helping to diagnose, attention-deficit/hyperactivity disorder (ADHD). Methods:We identified relevant studies and classified them using American Academy of Neurology criteria. Results: Two Class I studies assessing the ability of EEG theta/beta power ratio and EEG frontal beta power to identify patients with ADHD correctly identified 166 of 185 participants. Both studies evaluated theta/beta power ratio and frontal beta power i… Show more

“…Yet, despite the tremendous progress in computing power and available algorithms, the spectral band approach continues to persist as the dominant approach to EEG analysis, including in the development of clinical biomarkers. A recent example of this is the approval by the FDA 4 of the use of the theta/beta ratio as a biomarker for ADHD diagnosis (Saad et al, 2015; Gloss et al, 2016) whilst others are exploring the application of alpha-asymmetry as a potential marker for depression (van der Vinne et al, 2017; Kaiser et al, 2018). One question, therefore, is whether the approach of splicing the power spectrum into bands has persisted because it offers a superior approach in terms of research insight, methodological standardization, and reliability of results across studies, or whether it is because researchers have simply kept with the status quo of 80 years ago.…”

“…ADHD involves characteristic electroencephalographic (EEG) activity (Loo and Barkley, 2005 ). Electrophysiological markers that differentiate between ADHD patients and controls are based on three key analysis paradigms: (i) raw EEG analysis methods and tools (Arns et al, 2013 ), such as the NEBA LTD commercial tool evaluation based on the theta/beta ratio (Snyder et al, 2015 ; Gloss et al, 2016 ), which was approved by the Food and Drug Administration (FDA); (ii) event-related analysis methods, such as event-related potentials (ERP) and event-related synchronization/desynchronization (ERS/ERD; Fisher et al, 2011 ; Shahaf et al, 2015 ); and (iii) task-related analysis, which involves raw EEG analysis methods applied to samples collected during designated tasks, for example, CPT (Loo et al, 2009 ). Tasks such as CPT, often used in ERP analysis (Sunohara et al, 1999 ), are analyzed in a task-related manner, without synchronization to stimulus time.…”
