Predicting ongoing pregnancy chances after IVF and ICSI: a national prospective study

Abstract: The most important predictors of the pregnancy chance after IVF and ICSI are women's age and ICSI. The diagnostic category is of no consequence. Duration of subfertility and pregnancy history are of limited prognostic value.

“…40 -44 In comparison, a recent national Dutch cohort of 4928 women undergoing ART had an average age of 33.6 years and a mean duration of attempting conception of 3.6 years, with a 1-year cumulative probability of ongoing pregnancy of 45%. 35 Importantly, the duration of time attempting conception in our study is greater than of all the studies with which we compare in the remaining discussion, and the mean age of the women in our study is also greater than most of them.…”

“…In a recent populationbased cohort of couples receiving ART in the Netherlands, the cumulative pregnancy rate at 1 year was 45%. 35 Taken together, these results suggest that the overall success rates of treatment with NPT might be comparable to ART despite greater per-cycle pregnancy rates with ART. Although it may seem paradoxical that a treatment with a lower per-cycle pregnancy rate than ART may have a comparable cumulative rates of pregnancy or live birth, very similar results have also been found in randomized trials of ART versus other less invasive therapies for couples with unexplained infertility.…”

“…The patients' age, BMI and other known confounding factors such as duration of infertility, the serum levels of LH, FSH, TT and E 2 on the 3rd day of spontaneous menstrual cycle, the cycle length, the dosage of recombinant FSH administered, induction length, number of follicles, number of follicles with diameters of more than 14 mm, number of embryos transferred per cycle, embryo cleavage rate, good-quality embryo rate and fertilization rate have been well controlled in the present study. These factors have been shown important in large prospective studies 50,51 . The lifestyle factors such as smoking and alcohol intake were also well controlled in the study.…”

“…The use of exogenous gonadotropins in combination with GnRH analogues for controlled ovarian hyperstimulation (COH) raises the number of oocytes retrieved and thus improves the results of IVF procedures [2], with cumulative pregnancy rate being approximately 63 % after 4 cycles [3,4]. However, the use of COH is associated with serious clinical problems like multiple pregnancies [5] and ovarian hyperstimulation syndrome (OHSS) [6].…”
