Prediction of additional lymph node positivity and clinical outcome of micrometastases in sentinel lymph nodes in cutaneous melanoma: A multi‐institutional study of 450 patients in Japan

Abstract: Various microscopic classifications of metastatic sentinel lymph nodes (SLN) have been reported along with predictors of additional lymph node positivity and their correlations with the prognosis. The purpose of this study was to re-evaluate these classifications in the Japanese population. We selected the following three classifications, based on the procedural simplicity of the measurements: maximum diameter (maximum diameter of the largest tumor lesion in the SLN; <0.1, 0.1-1.0, >1.0 mm), invasion depth (de… Show more

“…Although being older than 65 years was found to be an independent prognostic factor of OS, gender and tumor localization were not associated with OS and disease-free survival (DFS) (Wu et al, 2013). However, another study reported that tumor location, gender and age were not correlated with DFS and OS (Namikawa et al, 2012). In another study from Japan, age and gender were not associated with DFS for patients with thick melanoma (Fujisawa et al, 2012).…”

“…There are fewer studies on SLNB from Asian countries compared with Western countries. The studies performed in Asian countries have revealed rates of SLN positivity ranging 15.5–41.8% . The rate of SLN positivity in this study was 42.2%, which is relatively high compared with previously obtained values.…”

“…The association between positive SLN status and DFS was not analyzed. Another recent multicenter, large-scale study in Japan showed that SLN metastasis is a prognostic factor for both OS and DFS20. In accordance with these results, our study showed that patients with positive SLN status had a higher local recurrence rate (42.9% vs.10.0%, p =0.042) and tended to show a higher rate of distant metastasis (50.0% vs. 20.0%, p =0.135) than those with negative SLN status.…”
