“…Nowadays, chemotherapy is administrated before or after radiotherapy, and the sequence of radiotherapy, chemotherapy and surgery has been extensively explored. In general, INCT combined with CRT is a preferred method since it is associated with better compliance and fewer acute adverse reactions (13,50,52). In our study, all patients underwent two cycles of induction chemotherapy and fluorouracil-based chemoradiotherapy.…”

“…Similar results were obtained in previous publications, highlighting the potential predictive role of body composition assessment and SMI in LARC patients undergoing RT [31] , [32] . One of the possible explications of this finding could be the relationship between lower muscle mass and the elevated inflammatory index, detrimental to clinical and survival outcomes [33] , [34] . In the CAO/ARO/AIO-04 clinical trial, performed on 1236 rectal cancer-affected patients, Fokas et al showed that underweight patients had worse DFS, was associated with worse TME quality and higher incidence of acute organ toxicity, and showed higher T stage and reduced ECOG performance status, giving the plausible explanation that underweight is linked to inflammation, a hallmark of cancer with muscle wasting and fat depletion induced by increased levels of cytokines [35] .…”

“…However, the present study suffers from some limitations: firstly, it is a retrospective analysis with a wide accrual period; consequently, there is a lack of information about the others prognostic factors such as EMVI status, the presence of tumoral deposits, mucinous histotype and biochemical parameters. Added value could come from combining radiomic information with that derived from lymphocyte profiles or other blood parameters, which have shown predictive power for radiotherapy response in other settings [ 50 ].…”
