Predictive factors for long-term survival after surgery for pancreatic ductal adenocarcinoma: Making a case for standardized reporting of the resection margin using certified cancer center data

Abstract: Factors for overall survival after pancreatic ductal adenocarcinoma (PDAC) seem to be nodal status, chemotherapy administration, UICC staging, and resection margin. However, there is no consensus on the definition for tumor free resection margin. Therefore, univariate OS as well as multivariate long-term survival using cancer center data was analyzed with regards to two different resection margin definitions. Ninety-five patients met inclusion criteria (pancreatic head PDAC, R0/R1, no 30 days mortality). OS wa… Show more

“…As a consequence of the lack of consensus that existed until recently, R1 rates published in the literature may be based on different R1 definitions, which, inevitably, has had an impact on the reported data. Indeed, several recent studies on cancers in the pancreatic head report that the R1 rate increases up to threefold if R1 is based on ≤1 mm clearance instead of 0 mm [ 27 , 28 , 29 , 30 ]. Similarly, for PDAC in the pancreatic body and tail, Hank and colleagues reported an R1 rate of 76.5% with invasion within 1 mm to the margin, and 53.6% with direct margin involvement (0 mm clearance) [ 31 ].…”

“…We began variable selection by including chemotherapy and LNR in our model, as they have been found to be the best predictors for long-term survival for pancreatic ductal adenocarcinoma. 36 It should be noted that for our observational data, the association between survival times and chemotherapy should be interpreted with care since treatment with chemotherapy might be subject to channeling bias. Treatment recommendations are usually given through an interdisciplinary tumor board.…”

“…Tumor free resection margins generally serve as a long-term prognostic factor [5,6]. A clear definition of resection margin status is therefore essential [7]. According to many centers in Europe, the R classification was adapted to Royal College of Pathologist (RCP) guidelines [8].…”
