Predictive power of cytomorphological features in equivocal (C3, C4) breast FNAC

Abstract: Definitive immediate diagnosis in breast fine needle aspiration cytology (FNAC) remains the aim for cytopathologists. We reviewed 72 consecutive equivocal (C3 and C4) aspirates with respect to 16 cytomorphological criteria. We assessed the power of each criterion at predicting either a malignant [positive predictive value (PPV)] or a benign [negative predictive value (NPV)] diagnosis by correlation with follow-up histology. Blind review led to 34% of cases being correctly definitively diagnosed. Eccentrically … Show more

“…Moyes and Dunne [24] tested utility of some cytomorphological criteria for atypical and suspicious cases discrimination. They demonstrated the predictive value, with about 80% specificity, of eccentrically placed epithelial cell nuclei and coarse nuclear chromatin for predicting malignancy.…”

“…The cellularity of each sample was quantified as low when there were <10 but >6 clusters observed per sample, as moderate when the number of clusters per sample ranged between 10 and 30 and as many when >30 clusters per sample were seen [11,12]. The predominant size of the cell clusters was recorded as small (3-15 epithelial cells/cluster), medium (16-75 cells/cluster) or large (>75 cells/cluster) [12].…”

“…C2 and C5 have high predictive values as shown by many studies. The suspicious C4 category is not as certain as C5 but still suggests a high probability of malignancy. The C3 category is an equivocal category commonly used when there is some diagnostic uncertainty regarding the true nature of the lesion (1)(2)(3)(4)(5). There are no standard clinical management strategies for C3 cytology.…”
