Predictive value of JC virus PCR in cerebrospinal fluid in the diagnosis of PML

Abstract: Objective: To assess the predictive value of JC virus (JCV) PCR in cerebrospinal fluid (CSF) in the diagnosis of progressive multifocal leukoencephalopathy (PML). Methods: We conducted a retrospective database query to identify patients with positive CSF JCV PCR. Clinical features, final diagnosis and quantitative PCR results were obtained. Results: A positive CSF JCV PCR had a PPV of 10.4% for the diagnosis of PML. A weakly positive PCR had a PPV of 1.6%, whereas a moderately to highly positive PCR had a PPV … Show more

“…Nevertheless, in immunocompromised renal allograft recipients, JCV can bring about nephropathy and/or PML [ 6 ]. PML occurs mainly in acquired immunodeficiency syndrome (AIDS) patients and less frequently after solid organ or allogeneic hematopoietic stem cell transplantations and in Multiple Sclerosis or Crohn's disease patients treated with immunomodulatory drugs [ 7 ].…”

“…1 JCV load from CSF-PCR may be of more importance than the qualitative result with significantly positive JCV loads (⩾2.7 log copies/mL) shown to confer to high likelihood of PML in those with clinico-radiological features of PML. 7 Demyelination, abnormal oligodendrocytes with giant nuclei, multinucleated astrocytes, foamy macrophages and reactive gliosis are typically seen in the brain specimens of patients with PML. 4,5 Analysis of brain tissue by methods of immunohistochemistry, PCR, electron-microscopy can also detect intraprenchymal JCV.…”

“…HSV encephalitis in these patients may present atypically and is associated with poorer outcomes [18,19]. Other viruses in immunocompromised population include cytomegalovirus (CMV), human herpesvirus (HHV)-6 and -7 as well as John Cunningham-virus (JCV) [20]. Non-viral causes of acute encephalitis include Mycobacterium tuberculosis, which may account for 5% of etiologies of acute meningoencephalitis [16].…”
