“…Complementary information at first-visit examinations included the lesional history (eg, increase in size, itching, scaling, change in color, intermittent bleeding), and the ugly duckling sign 25 and moles-breed-true concept. 26 Like two opposite sides of a coin, both concepts belong together. Although the ugly duckling sign helps to identify melanoma, the moles-breed-true concept implicates that benign common nevi in a given patient resemble each other.…”

“…The majority of melanomas (65%) had a 7-point checklist score of 3 to 5 points. However, 8 melanomas (4 invasive, 4 in situ) scored less than 3 points, and would have been missed without complementary information (eg, patient anamnestic data, ugly duckling sign, 25 moles-breed-true concept 26 ). As expected, there was a close correlation of the scoring and the percentage of melanomas among excised lesions (Fig 2, A).…”

“…The key point in the examination of individuals with multiple nevi is therefore the identification of his or her predominant nevus pattern (defined as the pattern seen in more than 30% of all nevi), which then permits the identification of atypical lesions that deviate from this pattern (recommendation 2). [2][3][4][5][6][7][8][9] Some studies have noted that patient-related factors influence the evaluation of an individual nevus as well as the predominant nevus pattern. Knowledge of these factors and their correlation with dermoscopic features are important for the correct diagnosis and management of nevi.…”

“…3,4 Most individuals have a predominant nevus pattern; therefore, the examination of all lesions is an essential step in the recognition process because it allows the identification of lesions deviating from the individual's prevailing benign pattern (concept of the "ugly duckling sign"). [2][3][4][5][6][7][8][9] By allowing visualization of submacroscopic pigmented structures that correlate with specific underlying histopathologic structures, 10 dermoscopy provides a more powerful tool than the naked-eye examination for clinicians to determine the…”

“…[43] This concept has also been referred to as "moles breed true" in that individuals tend to harbor a limited number of dermoscopic patterns in their nevi. [44] An extension of these observations is that any skin lesion with outlier dermoscopic features should be carefully examined. Figure 13.…”
