Preliminary dosimetric study on feasibility of multi-beam boron neutron capture therapy in patients with diffuse intrinsic pontine glioma without craniotomy

Abstract: Diffuse intrinsic pontine glioma is a very frustrating disease. Since the tumor infiltrates the brain stem, surgical removal is often impossible. For conventional radiotherapy, the dose constraint of the brain stem impedes attempts at further dose escalation. Boron neutron capture therapy (BNCT), a targeted radiotherapy, carries the potential to selectively irradiate tumors with an adequate dose while sparing adjacent normal tissue. In this study, 12 consecutive patients treated with conventional radiotherapy … Show more

“…BNCT has recently gained attention in salvage treatment for recurrent head, neck, and brain tumors. 12 , 13 In Japan, BNCT has been approved as a standard treatment modality and has been included in the national health insurance since May 2020. More and more BNCT specialist hospitals were built and are constantly leading development both in the number of patients treated and in basic research studies.…”

“…Also, in this study of single-field radiation, the greatest tumor depth modeled was 8 cm. For tumors at depths greater than the AD, better dose distribution may be achieved with multi-field BNCT [21]. Finally, the effects of positioning errors on multifield cases were not evaluated.…”

“…However, the issue of non-uniform dose distribution within tumors has consistently posed a challenge in the context of BNCT (Yu et al 2017). Multi-field neutron beams have the potential to enhance the dose distribution to large tumors (Fujimoto et al 2015, Lee et al 2017, which is similar to the principle of three-dimensional conformal radiotherapy (3D CRT). However, neutron beams are difficult to achieve intensity modulation within a single field considering its forward angle and field size.…”
