Preoperative endoscopic third ventriculostomy in children with posterior fossa tumors: an institution experience

Abstract: AIm:To assess the effectiveness and safety of pre-resection endoscopic third ventriculostomy (ETV) in permanently relieving hydrocephalus in children with posterior fossa tumors. mAterIAl and methOds: 17 pediatric patients with posterior fossa tumors and associated triventricular obstructive hydrocephalus underwent ETV before definitive tumor resection, and ETV was repeated after tumor resection if hydrocephalus with increased intracranial pressure persisted or recurred. The medical records, operative notes an… Show more

“…Ventriculitis is associated with persistent hydrocephalus after posterior fossa tumor surgery. 20,22,23 Fortunately, the patients in our series with CSF fistula and ventriculitis only required an EVD for 5 days. No further permanent CSF diverting procedure during the follow-up of 149 months has been necessary.…”

“…A higher rate of ETV/CPC failure was seen in grade IV hemorrhage, weight 3 kg, age < 3 months at the time of surgery, presence of a normal cerebral aqueduct, and a narrow prepontine cistern on MRI. 26 Although ETV should be done in simple cases in the initial learning curve, it can also be performed in posttraumatic hydrocephalus, 27 large or giant basilar artery aneurysms, 28 posterior fossa tumors, 29 and after shunt block 30 when sufficient experience is acquired. The shunt should be directly inspected during endoscopy; it should only be removed when there are no adhesions and neovascularization.…”
