Presacral Epidermoid Cyst with Right Hydronephrosis

Abstract: A presacral epidermoid tumor is a rare tumor and a large presacral cystic tumor may mimic an ovarian tumor in imaging. All presacral tumors with or without symptoms should be evaluated thoroughly before operation. Complete surgical excision, preserving adjacent organs' function, should be kept in mind.

“…Most of the symptoms are attributed to the mass effect of the cyst, which is governed by the cyst size. The most common complaints of the patient include constipation, pain and discomfort during defecation, and vague lower abdominal pain and hydronephrosis due to the mass effect [6]. Many of the cysts become symptomatic after superadded infection, which can result in chronic non-healing sinus, perineal fistulas, or bleeding per rectum [7,8].…”

“…Postoperative pathological examination (Table 1) showed that three cases (cases 9, 23, 39) had ruptured cystic lesions, three cases (cases 4, 23, 36) had foreign body reaction, which indicated a previous cyst rupture history [15], and five cases (cases 4, 9, 24, 33, 39) had infected lesions, which were likely to rupture over time. All of the above seven patients (cases 4, 9, 23, 24, 33, 36, 39) were female with epidermoid cysts.…”

“…Unlike malignancies in this area, they are not often associated with pain, change in bowel or bladder habits, or paresthesias. 21,22 However, some have reported symptoms such as vague rectal fullness or pain (perhaps worse in a sitting position or with ambulation), chronic constipation, and/or low back pain 13,14,16,17 In 1 study of 14 patients, 43% of patients were asymptomatic. 3 Another recent study of 34 patients revealed that 56% were asymptomatic, in comparison with only 14% of patients with malignancy that were asymptomatic.…”
