Primary Cardiac Angiosarcoma with Right Coronary Artery-to-Pericardial Fistula

Abstract: Primary cardiac sarcoma is a rare tumor that is difficult to diagnose preoperatively. Hemopericardium and coronary artery fistula are rare complications of primary cardiac sarcoma. The authors report a case of primary cardiac angiosarcoma presenting with hemopericardium, secondary to right coronary artery-to-pericardial fistula, with a review of the literature.

“…Hemopericardium, atrium rupture, and coronary artery fistula are very rare complications of primary cardiac angiosarcoma with <10 cases reported. [4][5][6][7] To the best of our knowledge, this is the first case of simultaneous rupture of right atrium and RCA. Theoretically, either of these disorders could cause sudden cardiac death, which, however, did not happen to this patient.…”

“…Etiologically, some rare pathologies can cause such mass lesions. For instance, hemopericardium and coronary artery fistula are rare complications of primary cardiac sarcoma [6]. Cardiac echinococcosis should be included in the differential diagnosis of cardiac mass lesions, particularly in patients originating from endemic areas [7].…”

“…Acquired coronary fistulas may be caused by blunt or penetrating trauma, or iatrogenically during coronary angiography [2]. Case reports have documented coronary fistulas into angiosarcomas [3] and even one coronary fistula into a pericardial hematoma, the cause of which could not be found [4]. All are rare occurrences.…”
