Primary hydatid cyst looked like renal cell carcinoma: case report

Abstract: Renal hydatid cyst is a rare disease comprising of about 2–3% of all locations. We present a case of renal hydatid disease in a 48-year-old female patient who presented with a history of left flank pain without fever or hematuria. Ultrasonography and computed tomography showed a 3-cm cystic lesion Bosniak IV occupying the mid-zone of the left kidney diagnosed as renal malignancy. Laparoscopic partial nephrectomy was performed, but the histopathological examination of the lesion revealed hydatid cysts. Isolated… Show more

“…The fact that the cysts were confined to the kidneys made differential diagnosis with renal cystic malignancy more difficult. Indeed, most of the time it is hard to distinguish a hydatid cyst from a cystic renal tumor based on radiological findings, as many other authors were confronted with this diagnostic problem [ 7 , 8 ]. ELISA is a sensible diagnostic test and is also useful in disease monitoring after surgical treatment, however inactive cysts or active cysts that are not eroded or leaking can illicit little to none immune response from the host resulting in a negative test [ 9 ].…”

“…Many cases of isolated RHCs being misdiagnosed have been reported and operated as renal tumors in the literature. [14][15][16] The challenge is to avoid the spillage of the content of the cyst.…”

“…(2) The patient had a history of living in a pastoral area in Xinjiang for many years, but the place of long‐term residence did not belong to the above‐mentioned high incidence area, so the diagnosis was easily missed; (3) In our case, the CT showed a cyst Bosniak IV according to the modified Bosniak Classification of Cystic Renal Masses, 13 which was difficult to differentiate from cystic renal cell carcinoma. Many cases of isolated RHCs being misdiagnosed have been reported and operated as renal tumors in the literature 14–16 . The challenge is to avoid the spillage of the content of the cyst.…”

“… With regard to their case ultrasound scan and CT scan demonstrated an atypical cyst that was classified as Bosniak IV according to the modified Bosniak classification of cystic kidney masses. Magnetic resonance imaging (MRI) scan is useful for assessing type IV Bosniak cysts in order to differentiate between pseudo-tumoral hydatid cysts and cystic renal carcinoma but MRI scan could be inconclusive [51].…”
