Primary Hyperparathyroidism in Pregnancy—A Rare Cause of Life-Threatening Hypercalcemia: Case Report and Literature Review

Abstract: Objective. To report a case of primary hyperparathyroidism in a pregnant patient, report the obstetric and neonatal outcomes, and review the relevant literature. Results. A 29-year-old primigravida was successfully treated for PHP with minimally invasive resection of a parathyroid adenoma in the second trimester of pregnancy. A healthy baby girl was delivered at 37-week gestation with an unremarkable neonatal course. To the best of our knowledge, this is the second case report in the literature utilizing intra… Show more

“…Total serum calcium declines during gestation, likely due to plasma volume expansion. The upper limit of normal for total serum calcium is 9.5 mg/dL in pregnancy (13). Parathyroidectomy is generally recommended for pregnant patients with calcium levels >11 mg/dL, particularly in patients with prior pregnancy loss (5).…”

“…First-trimester surgery is avoided due to incomplete fetal organogenesis, and third-trimester surgery is not recommended because it is associated with a higher risk of preterm labor and increased fetal mortality (3). Severe (calcium levels >12 mg/dL) or refractory hypercalcemia may require acute short-term hemodialysis or urgent parathyroidectomy, regardless of the gestational age (13).…”
