Purpose Groin herniorrhaphy is the most common operation performed by general surgeons. Annually, more than 20 million groin hernias are repaired worldwide. The general approach towards groin hernias is surgical repair regardless of the presence of symptoms. The rationale to recommend surgery for asymptomatic groin hernias is prevention of visceral strangulation. The goal of this review is to evaluate the appropriateness of surgery in patients with asymptomatic groin hernias. Methods The review was based on an extensive literature search of Pubmed, Medline and the Cochrane Library. Results The risk of incarceration is approximately 4 per 1,000 patients with a groin hernia per year. Risk factors for incarceration are age above 60 years, femoral hernia site and duration of signs less than 3 months. Morbidity and mortality rates of emergency groin hernia repair are higher in patients who are older than 49 years, have a delay between onset of symptoms and surgery of more than 12 h, have a femoral hernia, have nonviable bowel and have an ASA-class of 3 or 4. The recurrence rate after tension-free mesh repair in the management of emergency groin hernias is comparable to that of elective repair. There is no diVerence in pain and quality of life after elective repair compared to watchful waiting. There is no advantage in cost-eVectiveness of elective repair compared to watchful waiting. ConclusionWatchful waiting for asymptomatic groin hernias is a safe and cost-eVective modality in patients who are under 50 years old, have an ASA class of 1 or 2, an inguinal hernia, and a duration of signs of more than 3 months.

There are several studies on inguinal hernias that give some clues about the association between this type of hernia and certain risk factors. This association was suspected long ago but had not been demonstrated. The present study tries to correlate the origin of inguinal hernias and the physical effort of the subjects in accordance with their work activity. The results of this study show that physical effort, as a risk factor, is closely related to the appearance of inguinal hernias. A person whose work involves lifting or other strenuous exertion has a higher risk than those whose jobs are less strenuous (p less than 0.05). This conclusion was reached after taking into account not only the weight lifted but also the number of years in this activity.

In a retrospective analysis the late results--mean follow-up 13.2 years--after herniotomy and Bassini repair are presented. The results of 837 interventions were evaluated with a questionnaire and among these 341 were clinically controlled. The overall recurrence rate was 10.5% with a cumulative recurrence rate of 26% after the first two years and 88% within 10 years. A testicular atrophy was observed in 1.1% and in 1.6% patients complained of pain in the groin or presented an entrapment of the nervus ilio-inguinalis. The personally conducted clinical evaluation showed a recurrence rate of 23%, a testicular atrophy in 1.2% and an entrapment in 2.9%. The recurrence rate was independent of sex, age, localisation, type of hernia, increased intraabdominal pressure or experience of the operator. Our results confirm that recurrences are only depicted by a clinical guided control after a sufficiently long follow-up period.
