Primary multifocal skeletal Hodgkin’s disease confined to bone

Abstract: Clinical informationThe patient, a 63-year-old man, presented complaining of severe pain in the left thigh and marked weight loss. Haematological screening tests revealed a mild leucocytosis (WCCr = 16x103) and a markedly elevated erythrocyte sedimentation rate at 90 mm/h. An anteroposterior radiograph of the left hip revealed the presence of focal lyric bone destruction in the neck of the lemur (Fig. 1) corresponding to a site of increased uptake at scintigraphy (Fig. 2). Magnetic resonance imaging (MRI) conf… Show more

“…Three previously reported cases of primary Hodgkin disease involving multifocal osseous sites were treated with chemotherapy, with or without radiation 28–30. As in our examples of primary multifocal osseous Hodgkin disease, 2 of these cases29, 30 had limited follow‐up (less than 6 months); 1 survived without recurrence for 3 years 28…”

“…Most of the cases have been associated with synchronous lymph node involvement. Since 1927, only seven patients with primary solitary osseous HD [6,19,26,31,32,33] and five patients with primary multifocal osseous Hodgkin disease have been reported without lymphatic manifestations [13,16,29,31]. Two of these cases, which have been reported before 1944, presented limited clinical and radiographic evalu- ations.…”

“…In late stage HD, osseous localizations have been described in 10-20% of cases [3,10,17,20,24,30], with less then 2% of cases showing skeletal lesions at the time of initial presentation. To our knowledge multifocal primary osseous HD without lymphatic manifestations has been reported only in five cases [13,16,29,31]. This article describes one further case and reviews the literature with special emphasis on treatment and prognosis.…”

“…Primary bone involvement is very rare 294,295 with two cases of disseminated osseous disease recently reviewed. 296 Seven cases involving the mandible, including two primary cases, again highlight the rarity of this presentation. 297 Bone marrow Marrow disease is not that uncommon, particularly with advanced disease, with estimates of incidence of between 10 and 15 per cent.…”
