Primary non-secretory plasma cell leukemia with atypical morphology — a case report

Abstract: Only one case of primary non-secretory plasma cell leukemia with atypical morphology has been reported thus far. Here we report another such case of plasma cell leukemia diagnosed on fl ow cytometry, as morphological heterogeneity and lack of monoclonal immunoglobulins in both serum and urine, made it diffi cult to come to a conclusive diagnosis based purely on morphology.

“…Such morphological variations have been reported more commonly in plasma cell leukemia. These include monocytoid appearing plasma cells with cleaved nuclei, hairy cell-like cells, blastoid cells, and cases with lympho-plasmacytoid cells, few showing bipolar cytoplasmic projections (2)(3)(4). In our series, all the six cases had clinical features suggestive of plasma cell dyscrasias.…”

“…Immunohistochemistry, flow cytometric analyses and/or electron microscopic examination may be required to establish a definitive diagnosis. The diagnosis may be made with further difficulty in cases with a nonsecretory nature (7,9). Indeed, we did not initially suspect a diagnosis of PCL in patient 1 because most of the cells exhibited an atypical morphology, and no monoclonal proteins were detected in either the serum or urine.…”

“…Nonsecretory myeloma is also a rare form of MM characterized by the absence of monoclonal immunoglobulins on either serum or urine electrophoresis and represents less than 1% of cases of MM (4). Only a few well-documented cases of nonsecretory MM presenting as primary PCL have been reported (5)(6)(7)(8)(9). We herein report two elderly cases of primary nonsecretory PCL.…”

“…The exact incidence of pPCL is believed to be less than 1 case/million. [ 7 ] From the surveillance, epidemiology and end results (SEER) database it is evident that there are no significant differences based on gender, age, or race when compared with patients with MM. PCL occurs in all races and all geographic locations.…”

“…As with MM, PCL is more common in African Americans and blacks from Africa than in whites. [ 7 ] pPCL patients have a younger age at presentation when compared to MM or sPCL patients, and their performance status is usually worse and faster declining. [ 1 ] pPCL presents at a slightly younger age with median ages at diagnosis of 55 compared to patients with sPCL at 66.…”
