Primary pyomyositis: an unusual presentation in an older patient with no recognised risk factors

Abstract: Primary pyomyositis is a subacute deep bacterial infection of skeletal muscle. It is typically seen in tropical countries with Staphylococcus aureus being the commonest pathogen. Immunocompromised states and trauma are associated with cases in temperate climates where there is an increasing incidence, typically in children and young adults. However, the authors present a case of primary pyomyositis in a previously healthy 80-year-old female. The authors highlight the potential difficulties in early diagnosis o… Show more

“…CK may not be elevated as one would expect; this lack of CK elevation was seen in our patient 13. Furthermore, the microbiological diagnosis of pyomyositis can be difficult without biopsy as bacteraemia is present in only about 25–35% of patients 2…”

“…Risk factors for pyomyositis include trauma, poor nutrition, immunosuppression, HIV infection, diabetes, malignancy, rheumatological disease and cirrhosis 1 2 13. In temperate conditions the most salient risk factors appear to be HIV infection, malignancy, diabetes and steroid use 1 2 14 15.…”

“…Interestingly, low incidence of previous muscle trauma (0 to 23%) had already been reported in other series of bacterial myositis. 18,19 In this regard, an important clinical consideration is that all patients in our series presented relevant comorbidity and more than half of them suffered from at least two important extra-muscular diseases. Definite immunodeficiency status was registered in three patients; on the other hand, many of the observed chronic co-morbidities may rather be associated with various degrees of immunodeficiency.…”

“…Pyomyositis is an acute bacterial infection occurring in skeletal muscle with no obvious source that can lead to abscess formation . It is mainly found in tropical climates but is also found in temperate climates in patients who are immunocompromised – HIV, drug abuse, diabetes and trauma being the main predisposing factors . Diabetes is an important contributing factor to pyomyositis by predisposing the skeletal muscle to damage and increasing susceptibility to infections .…”
