Primary retroperitoneal echinococcal cyst

Abstract: A 74-year-old man was admitted with right flank pain and discomfort lasting for 2 months. CT scanning revealed a large retroperitoneal cystic mass. There were no cysts elsewhere. Serological testing revealed hydatid disease. Preoperatively he was treated by Albendazole 400 mg for 1 month and then underwent laparotomy. The entire mass was excised en bloc and intact and right hemicolectomy was simultaneously performed for excision of the adhered ascending colon. He recovered uneventfully and was discharged on th… Show more

“…Since our case was operated for the hydatid cyst in the liver, it was recognized that cysts in these two organs were primary cysts from the liver. The pre-operative diagnosis of cysts in our case was conducted with USG, MRI and CT consistent with the literature (3)(4)(5)(6)11,12) and the cuticular layer of the cyst was revealed with histopathological methods post-operationally.…”

“…There are reports in the literature on the secondary hydatid cyst cases in uterus and colon (10), ovary and colon (5) due to the perforation of the primary cyst in the liver, and ruptured to the right colon from the liver (4). There are also primary CE cases reported due to involvement of the uterus (11) and colon (12). Although, Tas et al (13), suspected an ovarian neoplasm in a 44-year-old menopausal patient with complaints of abdominal distention, they encountered primary ovary and omental CE during surgery.…”

“…While the presence of hydatid cysts in locations such as the liver and the lungs is anticipated, the observation of such cysts in the retroperitoneum without the involvement of any other organs is very uncommon. It would be wise to include hydatid disease in the differential diagnosis of a retroperitoneal cystic tumor, especially in cases where patients come from endemic areas [ 7 ]. The presence of hydatid disease in uncommon sites may be secondary, due to cyst rupture and spillage of its content, or primary through implantation via the lymphatic vessels of the gastrointestinal tract and the hepatic portal system [ 8 ].…”

“…The first primary retroperitoneal echinococcal cyst was reported in 1958 [10]. Since then, a few similar cases have been reported in the existing literature [7,[11][12][13][14]. In a retrospective study of 121 cases of retroperitoneal echinococcosis, 68 patients were treated surgically.…”

“…Cystic echinococcosis accounts for approximately 95% of all echinococcosis , while hepatic cystic echinococcosis accounts for approximately 75% of all cystic echinococcosis [ 3 ]. To date, there are numerous reports about single infections of cystic or alveolar echinococcosis in different organs[ 4 , 5 ], but there reports about mixed infections of cystic and alveolar echinococcosis in the same organ are rare. In this study, a typical case was chosen from the 4 patients with mixed infection of hepatic cystic and alveolar echinococcosis who were treated in the Division of General Surgery of Qinghai Provincial People's Hospital.…”
